摘要
①目的 探讨传统的根治性膀胱切除术对男性勃起功能的影响。②方法 对因膀胱癌行根治性膀胱切除术手术前性功能正常 2 8例男性病人 (或其配偶 )进行手术前后性活动问卷调查 ,分析手术后的勃起功能状况、性功能障碍的原因以及性功能障碍与病人年龄、肿瘤分期的关系。③结果 2 8例中 2 3例完全丧失性能力 ;2例保留一定的性活动能力 ;3例维持阴茎勃起 ,可以进行性交并获得性高潮 ,其接受手术时年龄分别为 36 ,43和 44岁 ,病理分期为T1期或T2 期。④结论 传统根治性膀胱切除术可以导致绝大多数男性病人勃起功能丧失 。
Objective\ To study the influence of radical cystectomy on male erectile function. \ Methods\ 28 male patients , with normal sexual function before operation, undergoing radical cystectomies from 1990 to 1999 were interviewed and asked the patients and/or their wifes to fill a questionnaire about their pre opreative and post operative sexual activity. The erectile function after operation, the causes of sexual dysfunction as well as its correlation with patients age, pathologic stage of the cancer at operation were analysed. \ Results\ 23 of 28 patients failed to achieve any form of sexual activity ;2 cases continued to have some sexual ability ; only 3 cases had penile erection sufficient enough to have intercourse and orgasm. The ages of the 3 cases at operation were 36, 43 and 44 years with the pathologic stage of T 1 or T 2 respectively. \ Conclusion\ Nearly all patients were impotent after classic radical cystectomies . The erectile dysfunction mainly arised from injury to the branches of pelvic nerve plexus cavernous nerves.
