肺功能动态监测对咳嗽变异性哮喘疗效评价 被引量:7

DYNAMIC MONITORING FOR PULMONARY FUNCTION IN THE TREATMENT OF COUGH VARIANT ASTHMA

摘要 ①目的 了解肺功能动态监测在咳嗽变异性哮喘疗效评价中的作用。②方法 对 42例咳嗽变异性哮喘病儿于发作期及缓解 0 .5 ,1.0年时进行常规肺功能及小气道功能检测。③结果 发作期肺功能各项指标明显异常 ,尤其是 5 0 %及 2 5 %肺活量时最大呼气流速与身高比 (V50 /Ht,V2 5/Ht)异常率达 92 .9% ,缓解 0 .5年时与发作期比较 ,其用力肺活量 (FVC)、第 1秒用力呼气容量 (FEV1.0 )、每分最大通气量 (MVV)、最大中段呼气流速 (MMF)、峰值呼气流速 (PEFR)有明显改善 (F =8.71~ 178.6 0 ,q=2 .83~ 9.37,P <0 .0 5 ,0 .0 1) ,但小气道功能损害仍明显 ,V50 /Ht,V2 5/Ht基本正常者仅 6例 (14.3% )。而缓解 1.0年时肺功能各项指标与 0 .5年对比又有明显改善 (q =2 .93~17.19,P <0 .0 5 ,0 .0 1)。小气道功能基本正常率达 73 .81% .④结论 咳嗽变异性哮喘经长期而系统的治疗后肺功能可以得到改善 。 Objective\ To probe the role of dynamic monitoring for pulmonary function in long term treatment of cough variant asthma. \ Methods\ We measured pulmonary and little airways function of 42 children with cough variant asthma treated in acute stage and in 0.5,1.0 year after remission. \ Results\ All parameters of pulmonary function in acute stage were obviously abnormal. After 6 months of remission , the pulmonary function was improved ( F=8.71-178.60,q=2.83-9.37, P <0.05, 0.01 ); but the damage of little airways function was still obvious in most of them, only 6 children were normal (14.3%); after 1 year of remission , all parameters of pulmonary function were much better than those in six months( q= 2.93-17.19, P < 0.05 ,0.01); the little airways function of 31 children was normal (73.81%).\ Conclusion\ This study demonstrates that little airways function of children with cough variant asthma can be improved by long term systemic treatment and dynamic monitor of pulmonary function is an effective way to evaluate the clinical efficacy.
