摘要
目的 探讨扩大前颅窝底入路切除颅鼻眶沟通肿瘤的效果 ,并对一些手术技巧加以改进。方法 采用经扩大前颅窝底入路对 5例颅鼻眶沟通肿瘤进行了手术切除。结果 肿瘤全切 3例 ,近全切 1例 ,大部分切除 1例。术后并发症 :脑脊液漏 1例 ,5例嗅觉均丧失 ,全组无手术死亡。结论 采用该入路切除颅鼻眶沟通肿瘤 ,具有显露好 ,颅底重建可靠等优点。突向中颅窝侧方较多的肿瘤则需结合颞部入路 ,对不能用颅骨内板行骨性重建者采用自体的额骨条重建 。
Objective To explore the operative results of removing cranionasal-orbital tumors and to modify some operative techniques. Methods The extensive transbasal approach was used in 5 patients with cranionasal-orbital tumors. Results Tumors were totally removed in 3 cases, nearly totally removed in 1 case and resected in most parts in 1 case. Postoperative complications: cerebrospinal fluid fistula in 1 case and disappearance of olfraction in 5 cases, but no one died in the surgery. Conclusion The procedure has the advantage of wide operative space and reliable reconstruction of the skull base. The removal of tumors close to the middle cranial fossa must be combined with the temporal approach. If the inner plate of the skull bone can not be used for the skull base reconstruction, the patient's own frontal bone slip can be used and the result proves better.
