甲基强的松龙及面罩无创机械通气救治危重哮喘的临床观察 被引量:5

摘要 目的 :探讨甲基强的松龙 (MPSS)冲击及加用面罩无创机械通气 (NPPV)救治危重哮喘的疗效。方法 :危重哮喘患者分三组 :(1 )地塞米松对照组 (DC组 ) :1 2例予静点地塞米松 1 0mg/次 ,2次 /d;(2 )MPSS治疗组 (MT组 ) :予MPSS静点 80mg/次 ,6h 1次 ,<72h ;(3)MPPS +NPPV治疗组 (MNT组 ) :予MPSS治疗同时予NPPV治疗。观察血气指标及临床症状改变。结果 :MNT组第 1h各指标迅速改善 ,第2 4h时各观察指标接近正常值范围。MT组第 1hPaO2 、PaCO2 明显改善 ,较DC组明显 (P <0 0 5) ,但各指标与MNT组相比仍存在差异 (P<0 0 5) ,第 2 4h各观察指标较治疗前明显改善 (P <0 0 5) ,但血气指标改善仍不如MNT组 (P <0 0 5)。DC组第 1h各指标无改善 ,第 2 4h除PaO2 及呼吸频率 (RR)有改善外 (P <0 0 5) ,余均无改善 ,且与MT组存在显著差异 (P <0 0 5)。结论 :甲基强的松龙冲击是救治危重哮喘的有效方法 ,如同时加用NPPV疗法则能迅速改善病情 ,是救治危重哮喘非常有效的手段。 Objective:To investigate the role of methylprednisone (MPSS) and noninvasive mask mechanical ventilation (NPPV) in emergency treatment of severe bronchial asthma.Methods:The patients with severe bronchial asthma were divided into three groups:(1)Dexamethasone treated group (DC group):12 patients were treated with intravenous dexamethasone 10 mg,2 times a day.(2)MPSS treated group (MT group):16 patients were treated with intravenous MPSS 80 mg,q 6 h.(3) MPSS+NPPV treated group (MNT group):14 patients were treated with MPSS and NPPV.The changes of blood gas analysis and clinical signs were recorded.Results:For MNT group,there was a significant response to intravenous MPSS and NPPV after 1 hour,and the parameters observed became normal after 24 hours.For MT group,PaO 2、PaCO 2 were significantly improved after 1 hour′s treatment,and there was a siginificant improvement of intravenous MPSS after 24 hours,but there were siginficant differences with MNT group in the improvement of the blood gas analysis after treatment (P<0 05).There was no any change with DC group after 1 hour′s treatment.Only PaO 2、respiratpry rate of DC group improved.But there were significant differences with MT group in other parameters (P<0 05).Conclusions:Methylprednisolone is an effective drug for emergency treatment of severe bronchial asthma.MPSS and NPPV may get severe bronchial asthma under effective control.
