摘要
目的 :探讨彩色多普勒血流显像近端等速表面积 (PISA)法测量二尖瓣狭窄瓣口面积 (MVA)的准确性。方法 :选取 38例二尖瓣狭窄病例 ,以手术中直接测量的二尖瓣口面积 (aMVA)为对照标准 ,对比分析PISA法、二维超声法 (2DE)、压差减半时间 (PHT)法、二尖瓣血流连续性方程 (MFCE)法的测值。结果 :PISA、2DE、PHT、MFCE法的MVA测值依次为 (1.2 4± 0 .5 6 )cm2 、(1.2 9± 0 .6 1)cm2 、(1.14± 0 .6 3)cm2 、(1.12± 0 .5 9)cm2 ,与aMVA(1 18± 0 5 7)cm2比较 ,除 2DE法P <0 0 5 ,其余P均 >0 0 5。 4种超声方法测值与aMVA的相关系数依次为 0 .89、0 .95、0 .71、0 .70。结论 :PISA法测量的准确性高 ,优于PHT法、MFCE法 ,略低于 2DE法 。
Aim: To explore the accuracy of the proximal isovelocity surface area (PISA) method in calculating valve area of mitral stenosis (MVA). Methods: MVA of 38 patients with mitral stenosis measured by PISA method, 2 dimensional echocardiography(2DE) method, pressure half time(PHT) method and mitral blood flow continuity equality(MFCE) method were analysed with the actual mitral valve area(aMVA) as control. Results: The results of PISA,2DE,MFCE were (1.24±0.56),(1.29± 0.61), (1.14±0.63) and (1.12±0.59)cm 2, and those measured by PISA and 2DE methods were closely correlated with aMVA(1 18±0 57)cm 2, r =0.89, 0.95 respectively). Conclusion: Compared with the PHT and MFCE method, the PISA method has relatively good accuracy.
