摘要
目的 分析结缔组织病 (CTD)重叠综合征 (OLs)的临床特点及肺功能等的变化 ,以探讨其早期对肺的损害。方法 分析经临床确诊的 37例OLs临床特点及静息肺功能的变化。结果 患者平均年龄 43.1岁 ,女性占 89.2 %。临床表现为关节肿痛占 1 0 0 % ,3/4的患者有雷诺现象 ,2 /3患者有皮肤硬化 ,约半数患者有晨僵 ,约 1 /3患者有呼吸系统症状。肺功能主要表现为PEF、FEF2 5 %～ 75 % 和DLCO下降 ,弥散功能障碍占 80 .0 % ,小气道功能障碍占 51 .4%。结论 OLs患者常有关节症状和皮肤病变 ,肺功能主要表现为弥散功能和小气道功能障碍 ,其中又以弥散功能下降发生率最高。
Objective To observe the clinical manifestations and static lung function of overlap syndrome (OLs) with connective tissue disease(CTD). Methods To analyze the clinical features and lung function test of 37 cases with clinic proved OLs.Results The mean age of patients was 43.1 years old. The percentage of female was 89.2%. All patients had swollen and painful joints, 3/4 Raynaud phenomenon, 2/3 sclerosis of skin,1/2 morning ragidity, and 1/3 respiatory symptoms. Clinic types: type I 40.5%, type Ⅱ 51.4%, type Ⅲ 8.1%. The characteristics of lung function were the decreases of PEF, FEF 25%～75% and DL CO . The percentage of diffusing capacity disorder was 80%, small airway dysfunction 51.4%. Conclusion Clinical features of OLs is coexsistence of PSS, SLE and RA, et al. The mean impairments of the lung function were the disorder of difussing capacity and small airway.
