注意缺陷多动障碍患儿α波竞争图的对照研究 被引量:12

A study on the alpha competitive structure of attention deficit hyperactivity disorder

摘要 目的 探讨注意缺陷多动障碍 (ADHD)患儿α波竞争图的特点。方法 运用脑超慢涨落分析技术和Conners问卷及韦氏智力测验 ,对 6 5例ADHD患儿 (ADHD组 )和 6 5名正常儿童 (对照组 )进行检测 ,比较两组的检测结果。结果  (1)ADHD组 8Hz成分高于对照组 (P <0 0 1) ,10Hz成分低于对照组 (P <0 0 1)。 (2 )ADHD组α波主频为 9Hz ,对照组为 10Hz ;ADHD组呈现双次频 ,即 8Hz和 10Hz ,对照组次频为 9Hz。 (3)ADHD组多数脑区熵值高于对照组 (P <0 0 5 )。 (4) 8Hz成分与Conners量表分呈正相关 (r =0 2 7~ 0 38,P≤ 0 0 1) ;10Hz成分与Conners量表分呈负相关 (r =- 0 2 1~ - 0 33,P≤ 0 0 1) ,与韦氏智力操作分、总分呈正相关 (r =0 35 5、0 336 ,P <0 0 5 ) ;熵值与Conners量表分呈正相关 (r=0 2 0~ 0 38,P <0 0 5 )。结论 ADHD患儿的α波慢化 ,脑有序度低 ,脑自组织能力差 ;其各种认知、行为症状与脑自组织能力差有关。 Objective To study the alpha competitive structure of children with attention deficit hyperactivity disorder (ADHD) Methods The study involved 65 children who met the DSM Ⅲ R diagnostic criteria of ADHD and 65 normal controls The subjects were assessed by encephalofluctuograph technology test, the Wechsler Intelligence Scale for Children and Conners′ behavior rating scale—Parent questionnaire Results (1)The dominant frequency component of alpha competitive structure in controls was 10 Hz, whereas it was 9 Hz in ADHD children The second component in controls was 9 Hz, while ADHD children had two secondary components, which were 10 Hz and 8 Hz respectively (2)Compared with control group, ADHD children showed significantly less 10 Hz activity and more 8 Hz activity (3)The energy distribution among alpha components was diffuse in ADHD, and the entropy value of ADHD children was higher than that of the controls (4)The dominant probability of 8 Hz was significantly positively correlated with the severity of symptoms, measured by Conners behavior scale scores ( r =0 27 0 38, P ≤0 01) The dominant probability of 10 Hz was significantly negatively correlated with severity of symptoms ( r =-0 21 -0 33, P ≤0 01), and was positively correlated with working and total IQ ( r =0 355,0 336, P <0 05) Entropy value was significantly positively correlated with Conners scale scores ( r =0 20 0 38, P <0 05) Conclusion ADHD subjects show slow and immature alpha pattern These findings indicate that ADHD children have poor brain self organization ability, which may underlie the symptoms
