心力衰竭病人细胞因子与心功能的相关性研究 被引量:6

A study of the relationship between cytokines and cardiac function in patients with congestive heart failure

摘要 目的 探讨充血性心力衰竭 (CHF)病人血中细胞因子浓度与心功能不全的关系。方法 采用双抗体夹心E LASA法检测 5 2例不同程度CHF病人和 30例正常人肿瘤坏死因子α(TNF -α)、白细胞介素 6 (IL - 6 )、可溶性肿瘤坏死因子受体I(S -TNF -RI)和转化生长因子 β(TGF - β)的血浓度 ,超声心动图测定左室舒张末内径 (LVEDd)、心输出量 (CO)、射血分数(EF)和心指数 (CI)。结果 心力衰竭组与正常组比较 ,血TNF -α、IL - 6、S -TNF -RI分别与LVEDd呈显著正相关 ,与CO、EF和CI呈显著负相关 (P <0 .0 5或 0 .0 1) ,而TGF - β血浓度与心脏扩大参数呈显著负相关 ,与心脏收缩功能参数呈显著正相关(P <0 .0 1)。结论 细胞因子变化与心力衰竭严重程度密切相关 。 Objective In order to explored the relationship between the blood concentration of cytokines and the ventricular dysfunction in patients with congestive heart failure(CHF).Methods Plasma levels of tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),soluble TNF receptor I (S-TNF-RI)and transforming growth factor(TGF)-β were measured by enzyme-linked immunoassay in 52 CHF patients with various degrees of heart failure and in 30 healthy controls.Left ventricular-end-duastolic diameter(LVEDd),cardiac output (CO),ejection fraction (EF)and cardiac index (CI) were examined with echocardiography.Results Plasma levels of TNF-α、IL-6、and S-TNF-RI were elevated in patients in functional classes Ⅱ to Ⅳcompared with age-matched control subjects and were progressively elevated in relation to decreasing functional status of the patients. Plasma levels of TGF-β were significant decreased in patients with CHF,particularly those in functional class Ⅳ.The levels of TNF-α、IL-6、S-TNF-RI respectively were positively correlated with LVEDd but negatively correlated with CO,EF and CI( P <0.05 or 0.01).The levels of TGF-β were negatively correlated with LVEDd but positively correlated with CO,EF and CI( P <0.01).Conclusion There were significant correlation between the plasma levels of cytokines and the severity of CHF.The dysregulated cytokine network may be involved in the development and progression of CHF. [
