摘要
目的观察异丙酚控制性降压对脑氧供需平衡的影响。方法择期颅脑手术患者30例,随机分为三组:Ⅰ组异丙酚降压组,Ⅱ组异丙酚常压组,Ⅲ组压宁定复合硝普钠降压组。Ⅰ、Ⅲ组分别于降压前、降压 20min、降压 40min、停止降压 20min(Ⅱ组同时点)同步采集桡动脉和颈内静脉血作血气分析,计算动—颈内静脉氧含量差(Da-jvO2)。结果Ⅰ组降压后Da-jvO2显著降低,而Ⅲ组Da-jvO2显著增加。结论异丙酚控制性降压期间脑组织氧供充分、氧耗减少,而压宁定复合硝普钠降压期间脑组织氧供相对不足、氧耗相对增加。异丙酚控制性降压能较好满足颅脑手术要求。
Objective To evaluated the effects of propofol - induced hypotension on balance of cerebral oxygen supply and consumption. Methods 30 patients undergoing elective procedures were randomly divided into three groups. Group Ⅰ, propofol - induced hypotension; Group Ⅱ, propofol - maintained normal arterial tension; Group Ⅲ, ebrantil combined with nitroprusside - induced hypotension. Before hypotension and after reducing hypotension 20min, 40min, stopped reducing hypotension 20min in Group Ⅰ and Ⅲ, radial arterial blood samples and jugular blood samples were taken simultaneously for blood gas analysis (in the same time, which were done in Group Ⅱ), and Da-jvO2 was calculated. Resultes In Group Ⅰ Da-jvO2 reduced significantly during induced hypotension, but increased significantly in Group Ⅲ. Conclusion Propofol - induced hypotension can increase cerebral oxygen supply and reduce oxygen consumption, but which is opposite to ebrantil combined with nitroprusside - induced hypotension. Propofol - induced hypotension is the more suited for brain tumor resection.
