摘要
目的探讨治疗颧上颌复合体骨折的理想方法和注意事项。方法通过头皮冠状切口径路 ,采用三维结构复位 ,小型钛接骨板坚固内固定术治疗颧上颌复合体骨折患者 46例。结果术后伤口均Ⅰ期愈合 ,有 1例面部畸形 ,1例咬合不正 ,1例张口受限 ,其余面部形态正常 ,无功能障碍。结论颧上颌复合体骨折的治疗以颧蝶缝、眶下缘的及早复位和颧额支柱、颧上颌支柱的坚固内固定尤为重要。植骨和重建骨性眼眶能有效地减少颧上颌复合体骨折的并发症。
ObjectiveTo explore a satisfactory method for the treatment of zygomaticomaxillary complex fractures and its items of paying attention. MethodsUsing technique of three dimensional space reposition and miniature titanium plate for osteosynthesis, forty-six patients with zygomaticomaxillary complex fractures were treated by surgical operation through head coronal incision.ResultsIn all patients, the surgical incisions healed by first intention after operation.But one of them had facial deformity, one had malocclusion, one had restriction of mouth opening; whereas the rest displayed symmetric facial contour and no functional disorders were found.ConclusionsEarlier reposition of zygomaticosphenoid fissure and infraorbital edge as well as the rigid internal fixation of zygomaticofrontal and zygomaticomaxillary pillars are very important in the treatment of zygomaticomaxillary complex fractures. Osteotransplantation and reconstruction of peri-orbital osteal malformation are effective for decreasing the complications of zygomaticomaxillary complex fractures.
