摘要
目的 探讨心电图(ECG)踏车运动试验ST/HR斜率及非ST/HR斜率参数对冠心病诊断价值。方法 对40例冠心病患者及30例正常对照者进行心电图踏车运动试验,测量ST/HR及非ST/HR斜率参数。结果 (1)各指标诊断冠心病的敏感性依次为:ST/HR斜率(90.00%),△QTC(84.40%),△Q(77.60%),△RST及△JTC(67.00%),△R(64.40%),ΣR(57.60%),ST(55.00%),U波倒置(31.00%),T波正常化(17.50%)ptfv1(12.50%),QRS时间延长(15.00%),心率指数及电轴偏移(30.00%)。特异性依次为:U波倒置(100.00%),ST/HR斜率及pftv1(97.50%),QRS时间延长(95.00%),T波正常化(92.50%),心率指数及电轴偏移(90.00%),ΣR(89.80%),ST及△JTC(83.20%),△R(79.42%),△RST(72.30%),△Q(65.20%),△QTC(61.60%)。(2)对上述指标进行Baye准则判别分析,筛选出诊断意义较大的指标组成或判别公式,其诊断冠心病的敏感性为92.00%,特异性为97.80%。结论 心电图多指标诊断冠心病明显优于单项指标。
Objective To evaluate the value of diagnosing coronary heart disease with electrocardiographic(ECG) mul-tiparameters. Methods Fourty Patients with coronary heart disease and 30 normal subjects were examined through ECG treadmill exercise test to measure ST segment and other parameters. Results (1)Sensitivities of all parameters for diagnosing coronary heart disease were as follows in order:ST/HR slope(90. 00%),△QTC(84. 40%),△Q(77. 60%),△RST and △JTC(67. 60%),△R(64. 40%),ΣR(57. 60%),ST(55. 00%),U wave inversion(31. 00%),T wave nomalization(17. 50%) ,ptfv1(12. 50%) ,QRS prolongation (15. 00%), rate index and electrical axis deviation (30. 00%). Specificities were listed below in order:U wave inversion (100. 00%),ST/HR slope and ptfv1 (97. 50%) ,QRS prolongation (95. 00%),T wave normalization (92. 50%),rate index and electrical axis deviation(90. 00%),ΣR(89.80%),ST and △JTC(83. 20%), △R(79. 42%),△RST(72.30%),△Q(65.30%),△QTC(61.60%). (2)by analysis of above parameters with Baye's principle,the more significant parameters for diagnosis were selected from all parameters to establish discriminant,by which the sensitivities and specificities in the diagnosis of coronary heart disease were up to 92.00%,and 97.80% respectively. Conclusion The results suggested that electrocar-diographic multiparameters have an advantage over single parameter in diagnosing coronary heart disease.
