大鼠重症急性胰腺炎病情演变中中性白细胞的作用 被引量:12

Pathogenic role of neutrophils in evolution of acute pancreatitis in rats

摘要 目的探讨中性白细胞在重症急性胰腺炎病程中的作用机制.方法将40只SD大鼠随机分四组;A组(单纯诱导模型组);B组(盐酸氮芥处理组);C组(假手术组);D组(盐酸氮芥处理的假手术组).各组动物8h后处死,取肺、胰腺标本观察病理变化.测定血清淀粉酶、血钙、BUN水平.用反转录聚合酶链式反应(RT-PCR)测定肺脏肿瘤坏死因子(TNFα)表达水平.结果假手术组胰肺组织结构正常.A,B组均发生急性胰腺炎,B组较之于A组胰、肺组织学病变(3.60±0.177)vs(9.08±0.348)、淀粉酶(5020±1512)U·L^(-1)vs(13919±3670)U·L^(-1),BUN水平(494±97.4)mg·L(-1) vs(828·99.5)mg·L^(-1)减轻(P<0.05.)A,B组均可检测到肺脏TNFαmRNA表达,其相对表达量B组明显低于A组(0.94±0.11)vs (1.81±0.22),P<0.05.结论过度激活的中性白细胞通过损伤自身组织、参与细胞因子的产生释放从而加重胰腺炎病情. AIM To investigate the role of neutrophil in the progression of acute psncreatitis. METHODS Forty SD rats were randomly assigned to group A with retrograde ductal infusion of sodium taurocholate exclusively; group B with methlorethemine administration; group C with sham operation and group D with methlorethamine administration + sham operation. Animals were sacrificed 8 h later. The severity of pancreatitis was determined by blind histologic grading and serum amylase, plasma calcium and BUN levels. The expression of TNF massenger RNA within the lung was established by the reverse transcription polymerass chain reaction (RT-PCR). RESULTS Acute pancrsstitis developed in both groups A and B. However, animals in group B showed significantly attenuated levels of serum amylase (5020 ± 1512 )U·L^(-1)vs (13919±3670) U·L^(-1) and BUN(494±97. 4)mg·L^(-1)vs (828 ±99. 5)mg·L^(-1) as well as lower histologic severity scores (3.60±0. 177) vs (9. 08± 0. 348), P<0. 05. TNF mRNA was uprogulatad in groups A and B (1. 81±0. 22 and 0. 94± 0. 11), respectively with significantly lower levels seen in group B (P<0. 05). CONCLUSION Activated neutrophil aggravates pancreatitis by causing tissue injury and cytokine secretion.
