摘要
目的 评价MRI对股骨头无菌坏死的诊断价值。方法 5 3位患者的 84个患病髋关节在拍摄X线平片一周内进行MRI检查。结果 0~Ⅰ期病变共计 2 5个髋关节 ,在MRI的分类中A类 17个 ,C类 8个 ;Ⅱ期病变共计35个 ,其中A类 17个 ,C类 17个 ;Ⅲ~Ⅳ期病变共计 2 4个 ,C类 12个 ,D类 7个。共计 2 2个髋关节出现“双线征” ,其中 0~Ⅰ期 13个 ,Ⅱ期 9个 ,Ⅲ~Ⅳ期未见此征象出现 ;2例病变在常规序列上未见异常 ,而STIR序列表现为小片状高信号影。结论 MRI与其他方法相比能够更早发现病变 ,STIR序列对与发现早期病变 (0~Ⅰ期 )有更高的敏感性。
Objective The diagnostic value of AVN of MRI were assessed. Methods MR images of 84 hips of 53 patients were obtained within 1 week of plain radiographs. Results Stage 0-Ⅰ lesions were 25 hips, 17 hips of class A, 8 hips of class C; Stage Ⅱ lesions were 35 hips, 17 hips of class A, 17hips of class C; Stage Ⅲ-Ⅳ lesions added up to 24 hips, 12 hips of class C, 7 hips of class D. There were 22 hips displaying 'double line' sign, 3 hips of stage 0-Ⅰ, 9 hips of stage Ⅱ, which were no seen in the stage Ⅲ-Ⅳ. The lesions of two cases are normal in the SE sequence, but are a small patch of high signal intensity in STIR. Conclusion MRI is more early to find lesions than the other techniques. STIR sequence is more sensitive than SE sequence. [
