腮腺部分切除术后再生与代偿的动物实验研究 被引量:17

摘要 [目的]研究腮腺组织再生和大涎腺代偿性改变的现象 ,探讨腮腺部分切除术的临床新应用。[方法]90只雌性SD大鼠的右侧腮腺分别行主导管结扎术、主导管结扎加部分腺体切除术和保留主导管的部分腺体切除术(各30只)。对照组设30只大鼠。术后1天、3天、1周、2周、4周、7周分别处死各组各5只实验动物。取双侧腮腺和双侧颌下腺制片。体视学方法测量腺泡、导管、间质三种成份的平均体积比和腺泡面积。[结果]结扎主导管组 :腺泡体积比和面积渐进性下降 ,直到最后(7周)完全被间质取代 ,导管系统体积比的下降缓慢而持久 ;结扎主导管加部分腺体切除术组 :最终结果与结扎主导管组一致 ,但存在有短暂的腺泡体积比的上升(2~4周) ;保留主导管的部分腺体切除术组 :腺泡体积比开始时下降(1~2周) ,随后上升并伴有腺泡面积增大 ,最终(7周)各组份体积比与对照组无显著性差异 ;三种手术后的其它大涎腺的各组份体积比和腺泡面积变化不大。[结论]行部分腺体切除术后的腮腺确有再生的能力。而主导管的保存与否 ,决定了再生是完全性的还是一过性的。在三种组成成份中 ,导管系统是最稳定的。保留导管的腮腺部分切除术有临床应用前景。本实验未发现其它大涎腺显著的代偿性增生的表现。 To study the regenerative characteristic and compensatory phenomena of salivary glands,the mechanisms and clinical application.One hundred and twenty female Sprague Dawley rats were used,and 30 served as controls.On the right parotid gland,the other 90 animals received three different operations:ligation of the Stensen's duct,partial parotidectomy and their combined way(30 rats in each group).Spe_cimens were taken post operatively on the 1st,3rd day, 1st,2nd, 4th and 7th week respectively.Stereological study was carried out on slides of bilateral parotid and submandibular glands.Proportional volume(PV) of the component tissues(acini,duct system and connective tissue)and acinar area(AA)were measured. 1.Group of the Stensen's duct ligation:The PV of acini and the AA decreased gradually until all acini were totally replaced by connective tissue.The PV of duct system decreased too,but more slowly;2.Group of partial parotidectomy plus ligation of Stensen's duct:the final results are the same as those of the Stensen duct ligation group,but presented transient increase of PV of acini;3.Group of partial parotidectomy without ligation of Stensen's duct;PV of acini decreased 1~2 weeks after operation,but it slightly increased in 2~4 weeks;finally there was no difference compared with the controls;4.There were no noteworthy stereological changes of the other salivary glands.[Conclusion]The parotid gland undergoing partial parotidectomy does have the ability of regeneration.But whether the Stensen's duct preserved or not determines a complete or transient regeneration .Of all the three components of gland,duct system is the most stable tissue. These results indicate that the application of partial parotidectomy without ligation the Stensen's duct might have clinical significance. There are no obvious compensatory changes of the other salivary glands in the experiment.
