摘要 目的 观察 2型糖尿病早期肾小管功能损害程度。方法 采用双抗体夹心酶免法对 71例 2型糖尿病患者进行尿微量白蛋白 (A1b) ,尿视黄醇结合蛋白 (RBP)和尿α1 微球蛋白 (α1 MG)排泌值测定 ,根据A1b排泌值分为尿Alb排泌正常组 (DM1)和尿A1b排泌异常组 (DM2 ) ,并与 40例正常健康者对照。结果  71例 2型糖尿病尿RBP和尿α1 MG排泌值都明显高于对照组 ,DM1组与对照组比较此两项排泌值均增多 ,DM1组排泌值较DM2 组有所增加 ,且DM1组中有 8例 (2 8.6 % )尿RBP升高和 5例 (17.8% )尿α1 MG增高。结论 在 2型糖尿病早期就可能存在肾小管功能损害 。 Objective To observe the damage of renal tubular function in the early type Ⅱ diabetes mellitus patients. Methods The urinary ALB/CR ratio, RBP/CR ratio and α1 MG/CR ratio in 71 cases of type t Ⅱ diabetes mellitus were measured by ELISA. All the patients were divided into normal urinal ALB/CR group (DM1) and abnormal urinal ALB/CR group (DM2) according to the ALB/CR ratio, and compared with 40 control subjects. Results Both urinary RBP/CR ratio and α1 MG/CR ratio in 71 patients with type Ⅱ diabetes mellitus increased markedly as compared with those in the control group. Both of them were significantly higher in DM1 group than those in the control group. And both of them in DM1 group were found to be higher than those in DM2 group. The urinary RBP/CR ratio was found to be increasing in 8 cases of DM1 group (28.6%) and the urinary α1 MG/CR ratio increasing in 5 cases of DM2 group (17.8%). Conclusion Renal tubular function probably has been damaged in early type Ⅱ diabetes mellitus. The abnormality of renal tubular might even appear before the damage of glomeruli.
