摘要
目的 探讨急性一氧化碳中毒(ACOP)对大鼠血管内皮及其细胞功能的影响和高压氧治疗(HBOT)的作用。方法 参照Jiang式染毒法制成ACOP大鼠模型,有相应对照地、分别对中毒后即刻和次日起每日1次HBOT的第1,3,7,14天后、血中一氧化氮(NO)、内皮素-l(ET-1)水平和循环内皮细胞(CEC)计数进行测定。结果①ACOP后即刻机体血中NO水平显著降低,ET-1水平和CEC计数显著升高(P<0.05);②HBOT后中毒大鼠血中NO水平显著回升(P<0.05),ET-1水平和CEC计数显著回降;其后上述指标都逐渐趋于恢复到正常水平。结论 ①ACOP可导致机体严重的血管内皮及其细胞功能的损伤;②0.2 MPa(绝对压)HBOT对ACOP导致的血管内皮及其功能损伤具有显著的改善和修复作用;③血中NO、ET-1水平和CEC计数指标的单独或联合检测,可作为临床上判断ACOP患者血管内皮及其功能损伤程度、评估疗效、预测预后的重要手段。
Objective To investigate the influence of acute carbon monoxide poisoning (ACOP) on vascular endothelium and the effect of hyperbaric oxygen (HBO) therapy in rats. Methods ACOP rat model was made by Jiang's technique. Then all the model rats (n = 72 ) were divided into two groups: HBO therapy group (n = 36) and control group (n = 36). In the HBO therapy group, all the rats were given HBO therapy immediately after poisoning and then once a day for 14 days consecutively. Blood levels of nitric oxide(NO) and endothelin-1 (ET-1) and circulating endothelial cell (CEC) counts were measured with blood taken immediately after poisoning and at the 1st, 3rd, 7th and 14th day after poisoning in each group respectively. At the same time, blood samples from 7 normal rats without CO poisoning were also measured as control group. Results 1. The blood levels sof NO decreased while ET- 1 and CEC count increased all significantly immediately after ACOP and were much lower than that of the control (P<0. 05). 2. After HBO therapy of the ACOP rats, the blood level of NO increased re- markable to normal level as compared with that of the control(P<0.05); and the blood level of ET-1 and CEC counts both decreased gradually to normal. Conclusions 1. ACOP could cause severe damage on the structure and function of vascular endothelium, which may play an important role in the patho- genesis of ACOP. 2. 0. 2 MPa HBO therapy could improve remarkably the oxygen supply, and thus quickly promote the recovery of injured vascular endothelium. 3. The blood levels of NO, ET-1 and CEC counts are valuable for the assessment of the severity of vascular endothelial damage and the effect of HBO therapy as well as for the predict of prognosis.
