摘要
目的 探讨舰船环境对口腔幽门螺杆菌（Ｈ．ｐｙ１ｏｒｉ）孳生的作用和对消化性溃疡（ＰＵ）的影响。方法 对舰船患ＰＵ者６０例和非ＰＵ者６０例进行口腔卫生调查，同时对其使用的牙刷和口腔牙菌斑进行Ｈ．ｐｙｌｏｒｉ－ＰＣＲ检测，以６０名陆地人员作为对照。结果 口腔卫生ＰＵ组忧良率仅３８．３％，明显低于非ＰＵ组（７０．０％）和陆地组（７３．３％）（Ｐ＜０．０５）。Ｈ．ｐｙｌｏｒｉ－ＰＣＲ检测发现，牙刷Ｈ．ｐｙｌｏｒｉ阳性率舰船两组分别为５６．７％和４８．３％，明显高于陆地组３５．０％（Ｐ＜０．０５）；牙菌斑Ｈ．ｐｙｌｏｒｉ阳性率三组分别为８ｌ．７％，３１．７％和３６．７％，ＰＵ组明显比其他组高（Ｐ＜０．０１），非ＰＵ组与陆地组无显著性差异（Ｐ＞０．０５）。结论 舰船环境适于Ｈ．ｐｙｌｏｒｉ在牙刷上孳生，Ｈ．ｐｙｌｏｒｉ在口腔的聚集受口腔卫生的影响，ＰＵ的发病可能与口腔Ｈ．ｐｙｌｏｒｉ有关，舰船环境可能是船员ＰＵ发病率和复发率高的一个重要原因。
Objective To verify the effects of navy vessel condition on oral helicobacter pylori (H. pylori ) and peptic ulcer (PU). Methods 1 462 male naval vessel personnel were surveyed for stom- ach diseases. 964 of them with gastric symptoms were given gastroscopy and 134 were found to have PU. Then 60 PU patients, 60 non PU naval vessel personnal and 60 healthy land personnel randomly selected as 3 groups were investigated and compared their oral hygiene and oral health habit. H. pylori in their tooth brush and tooth plaque were examined by polymerase chain reaction (PCR). Results The rates with best and fine oral hygienic condition of the 3 groups were 38.3%, 70.0% and 73.3% respec- tively and were different with statistical significance (P<0.05). H. pylori positive rate of tooth brush of the PU group was 56.7%, much higher than that of the land group (35.0%) (P<0.05), and the rate of non PU group(48.3% ) was also higher than that of lhe land group(P<0.05). H. pylori positive rates of tooth plaque of the 3 groups were 81.7%, 31.7% and 36.7%, respectively and were highly different statistically (P<0,01), but there was no statistical difference between the non PU and land groups (P >0.05). Conclusions Naval vessel condition could favor the growth of H. pylori on tooth brush, and bad oral hygienic condition might help the H. pylori colonization in mouth which may be related with PU morbidity. Naval vessel condition could be the important cause of high incidence and reccurence of PU in navy vessel personnel.
