摘要
目的 :评价角膜厚度在高眼压症眼压测量中的作用影响。方法 :选取 34例 (6 8眼 )高眼压症患者 (眼压≥ 2 1mm Hg,≤ 35 mm Hg,正常视野 ,正常视神经乳头 ,无角膜水肿等病变 )均为复旦大学医学院眼耳鼻喉科医院眼科门诊患者 ,年龄 6~ 6 5岁 (19.70±13.5 2 ) ,平均 2 0岁 ,男性 18例 ,女性 2 0例。由专人应用 Goldmann压平眼压计测量眼压 ,由专人应用 Sonoscan Model40 0 0 APA型超声测量中央角膜厚度。结果 :34例 6 8眼眼压范围 2 1~ 35 mm Hg,平均 2 4.892± 2 .885 mm Hg;中央角膜厚度 0 .5 2 3~0 .6 37mm,平均 0 .5 88± 0 .0 2 8mm。应用 SPSS软件包经 K—S检验二组数据 P值分别为 0 .30 6 1、 0 .842 2均呈正态分布 ,应用Pearson s相关统计法 ,二组数据呈正相关 ,相关系数为 0 .2 82 4,P=0 .0 2 (P<0 .0 5 )。根据本组病例可得出一相关方程式 :Y(眼压 ) =42 .40 1X(中央角膜厚度 )。即每 0 .0 2 36 mm的中央角膜厚度变化可有 1mm Hg的压平眼压值改变。结论 :中央角膜厚度与眼压测量值呈正相关 ,高眼压症患者眼压值的升高与中央角膜厚度的变化有关 ,揭示高眼压症患者的真正眼压可能是正常的 ,只是由于中央角膜厚度的变异使眼压测量值偏高。
Objective:To evaluate the effect of central corneal thickness (CCT) on applanation tonometry of ocular hypertension (OHT). Methods: Thirty-four cases(68 eyes) with OHT (21mmHg≤intraocular pressure[IOP]( 35mmHg) were all out-patients. Ages were from 6 to 65 years old (19.70(3.52). Men were 18 cases, women 20 cases. IOP was measured by Goldman applanation tonometer. CCT was determined with A type ultrasonography (Sonoscan Model 4000AP). Results: The mean ( SD of CCT in patients with OHT was 0.588 ( 0.28mm, the IOP was 24.892(885mmHg. According to the K-S test by SPSS soft, P values in the two groups were 0.3061 and 0.8422, respectively and both in normal distribution. Application of person's correlation statistic analysis showed that there was a positive correlation between the two groups, correlation coefficient was 0.2824, p = 0.02 (p<0.05). Correlation equation, Y=42.401X, was attained in term of the data. It meant that applanation tonometry is over/under estimated by approximately l mmHg for every 0.0236mm in CCT. Conclusion: The IOP is positively correlated with CCT. Variation of CCT results in elevation of IOP of patients with OHT, but the real IOP may be normal.
