摘要
目的 :探讨鼻内镜手术在非鼻窦炎息肉状病变方面的应用。方法 :利用鼻内镜手术治疗脑脊液鼻漏、脑膜脑膨出、蝶筛窦囊肿、异物、霉菌病、垂体瘤、后鼻孔闭锁、内翻性乳头状瘤共 70例。结果 :6 9例均一次手术成功。 1例脑脊液鼻漏再次手术成功。在 6个月～ 6年的随访中 ,仅后鼻孔闭锁及蝶窦霉菌各 2例在术后曾出现肉芽 ,经微波治疗后痊愈。结论 :鼻内镜能广泛应用于非鼻窦炎息肉状病变的治疗 ,脑脊液鼻漏、脑膜脑膨出是其最佳适应证 ;对其它手术也能体现出其进路简便、创伤小、直视下操作、视野清晰、不留颅面部畸形等优点。
Objective:To evaluate endoscopic surgery of non sinusitis nasal polypoid lesions.Methods:Seventy patients with cerebrospinal fluid rhinorrhea,meningoencephalocele,sphenoida and ethmoidal mucoceles,sphenoidal foreign body and fungi,pituitary tumor,choanal atresia,inverted papilloma were treated by endoscopic surgery.Results:One patient with cerebrospinal fluid rhinorrhea underwent reoperation because of recurrence.The other 69 cases were cured by one stage surgery and had no recurrence developed within 1/2～6 years follow up.Conclusions:Endoscopic surgery can be applied to all non sinusitis nasal polypoid lesions.It is the best choice of management for cerebrospinal fluid rhinorrhea and meningoencephalocele.This technique has advantage of clear field of vision,easy approach,fewer complications,mild damage and leaving no malformation.
