摘要
目的 :探讨双吻合器在低位直肠癌手术治疗中应用的适应证及并发症的预防。方法 :回顾性分析 1997年 11月至 2 0 0 0年 11间采用双吻合器治疗的低位直肠癌 146例的疗效 ,复发情况及并发症的发生与预防情况。结果 :146例低位直肠癌应用双吻合器进行 Dixon手术的效果良好 ,仅 2例局部复发 ,发生吻合口漏 2例 (1.4% ) ,吻合口狭窄 8例 (5 .4% ) ,术后早期吻合口大出血 4例 (2 .7% )。结论 :双吻合器的应用可以扩大 Dixon手术适应证的范围 ,但必须严格掌握根治原则及无瘤原则 ,否则易招致失败 ,在应用过程中 ,必须注意吻合口漏及吻合口狭窄的预防 。
Objective: To investigate indication and the prevention of complication in using double anastomat in low rectal cancer operation Methods:Retrospectively analyzing the effect,relapse stat,complication and the prevention of 146 low rectal cancer cases,which was anastomosised by double anastomast from Nov.1997 to Nov 2000 in our hospital Results:The Dixon operation effect is perfect in those 146 cases ,except 2 cases with local relapse,2 cases with stoma leak (1 4%),8 cases with stoma constriction(5 4%),4 cases with stoma hematorrhea after operation(2 7%) Conclusion:It can expand the indication's range in Dixon operation by using the double anastomat,but we must hold the eradication doctrine and non neoplasia doctrine to prevent the failure We must pay attention to the prevention of stoma leak and stoma constriction,especially the rare cases of stoma hematorrhea
