摘要
目的 :研究 50例风湿性心脏病二尖瓣狭窄 (风心二狭 )患者PBMV治疗前后血浆NO、ET及血流动力学变化。结果 :NO浓度术前及术后分别为 (10 8.350± 33.6 14)、(73.4 6 2± 2 3.913) μmol/L ;ET浓度术前及术后分别为 (130 .6 32± 4 3.2 84 )、(10 1.147± 31.90 4 )ng/L。NO、ET浓度术后明显低于术前 (P <0 .0 1)。术前MVA、LAD、mLAP、mMPG、mPAP分别为 (1 0 2 5± 0 .2 0 1)cm2 、(5 158± 0 548)cm及 (2 76 6± 0 597)、(1 74 7± 0 72 9)、(5 6 0 1± 0 718)kPa ;术后分别为 (1 715± 0 2 4 8)cm2 、(4 893± 0 585)cm及(1 734± 0 4 18)、(0 86 3± 0 4 15)、(4 156± 0 4 36 )kPa。术后血流动力学较术前有明显改善 (P <0 .0 5) ,血浆NO浓度与mLAP、mPAP呈正相关 (r分别为 0 56 4、0 4 53,P <0 .0 1) ;血浆ET浓度与mLAP、mPAP呈正相关 (r分别为 0 52 7、0 52 8,P <0 .0 1) ,且NO、ET浓度与心功能有关。结论 :NO、ET可作为判断风心二狭病情和PBMV疗效及心功能的良好指标。
Objective:To observe the changes of the plasmic nitric oxide(NO),endothelin(ET) and hemodynamics before and after PBMV of 50 patients with rheumatic mitral stenosis.Methods:The level of plasmic NO and ET in patients after PBMV and before PBMV was studied by chemical colorimetric analysis and radioimmunoassay.The MVA,LAD,mMPG,mPAP and mLAP were measured by two dimensional color Doppler and cardiact catheter.Results:The level of plasmic NO in patients was (108.350±33.614)μmol/L before PBMV and was (73.462±23.913)μmol/L after PBMV.The level of plasmic ET in patients was (130.632±43.284)ng/L before PBMV and was (101.147±31.904)ng/L after PBMV.The MVA,LAD,mLAP,mMPG and mPAP were ( 1.025 ±0.201)cm 2,(5.158±0.548)cm,(2.766±0.597),(1.747±0.729),(5.601±0.718)kPa respectively before PBMV and were (1.715±0.248)cm 2,(4.893±0.585)cm,(1.734±0.418),(0.863±0.415),(4.156±0.436)kPa respectively after PBMV.The plasmic level of NO was positively related with mLAP and mPAP(r=0.564,r=0.453 respectively,P<0.01);The plasmic levels of ET were positively related with mLAP and mPAP(r=0.527,r=0.528 respectively,P<0.01).Conclusion:The level of plasmic NO and ET is related to the degree of heart failure.It may be an indicator of condition and curative effect of PBMV.
