三维DSA与二维DSA在颅内动脉瘤诊断中价值的比较 被引量:27

摘要 目的 比较三维 (3D)DSA与二维 (2D)DSA在颅内动脉瘤诊断中的价值。方法  2 2名疑为颅内动脉瘤的患者同行 3DDSA和 2DDSA检查 ,双盲法诊断 3DDSA和 2DDSA分别发现的动脉瘤的个数、动脉瘤与载瘤动脉和邻近血管的关系是否清楚、动脉瘤囊内有无穿支。结果 发现的 30个动脉瘤中 3DDSA的假阳性数和假阴性数均为 0 ;2DDSA的假阳性数和假阴性数均为 2个。 3DDSA对 30个动脉瘤和载瘤动脉及邻近血管的关系均显示清楚 ,发现 2个动脉瘤囊内有穿支 ;2DDSA仅对 8个动脉瘤 (其中 4个后交通动脉瘤 ,3个前交通动脉瘤和 1个颈眼动脉瘤 )和载瘤动脉及邻近血管的关系显示清楚 ,无法证实动脉瘤囊内有无穿支。结论  3DDSA在颅内动脉瘤的诊断方面明显优越于 2DDSA。 Objective To compare the value of 3D DSA with 2D DSA in the diagnosis of intracranial aneurysms. Methods 22 patients suspected to be with intracranial aneurysms were undergone both 3D DSA and 2D DSA. The number of aneurysms, the relationship between aneurysm and parent artery、aneurysm and local vessels branches from the sac were detected with both 3D DSA and 2D DSA with double blind methods. Results No false positiveness and false negativeness of 3D DSA were found in diagnosis of 30 intracranial aneurysms while 2 false positiveness and 2 false negativeness of 2D DSA were found. 3D DSA could clarify the relationships of all aneurysms with their parent arteries and with the local vessels, and some branches from 2 sacs were identified with 3D DSA. 2D DSA could clarify the relationships of only 8 aneurysms (including 4 PcoAN、3 AcoAN and 1 CaroOphAN) with their parent arteries and with the local vessels, and 2D DSA could not identify the branches from the sac. Conclusions 3D DSA was superior to 2D DSA in the diagnosis of intracranial aneurysms.
