摘要
目的:探讨硬膜外麻醉侧卧位手术及辅助用药时血氧饱和度的变化。方法:60例硬膜外麻醉行侧卧位手术病人随机分成芬氟组及安定组,用无创搏动氧饱和度测定法,观察其硬膜外麻醉后、侧卧位后及辅助用药后sPO_2变化。结果:硬膜外麻醉后其sPO_2下降,但在正常范围,侧卧位后sPO_2下降明显(P<0.05),应用辅助用药后其sPO_2下降非常明显(P<0.01)。芬氟组sPO_2下降时间比安定组长,对呼吸抑制较安定组明显。结论:低氧血症之原因是硬膜外麻醉、侧卧位对呼吸功能的影响和安定、芬太尼氟哌啶合剂抑制呼吸复合作用的结果,故应用本麻醉方法时应常规吸氧,以免病人发生缺氧。
Objective: To discuss the effects on epidural anaesthesia lateral position and auxiliary medicines on sPO2 . Methods: In order to look into the change of sPO2 after epidurally anaesthizing, taking lateral position and auxiliary medicines, 60 Cases ;ire divided into two groups at random on auxiliary medicines with pelhidine-droperidol(GI) or diagepam (GII), using non-invasive examination. Results: sOP2 tends to drop some what after epidurally anaesthizing and drops notable in lateral psoition(pWTBX P<0.05), after taking auxiliary medicines sPO2 drops the most dramatically. It was different from auxiliary medicines, droping time of sPO2 was longer and the respiratory restraint was notable in G I . Conclusions: The hypoxemia results from eombined effects of epidural anaestkesia lateral position and auxiliary medicines. So if using this method, the patients should routinely inhale oxygen so as to avoid accident for want of oxygen.
