摘要
目的 :探讨经胸管回收自体血治疗胸外伤失血性休克的可行性。方法 :选择 1996年 1月至 2 0 0 0年 7月 46例胸外伤失血性休克患者 ,经胸管回收胸腔积血进行抢救。术中用足量广谱抗生素一次 ,术后常规应用抗生素。结果 :经胸管回收自体血 80 0～ 15 0 0 m l者 2 2例 ,15 0 1～ 2 0 0 0 ml者 17例 ,大于 2 0 0 0 ml者 7例。 46例患者除 1例心脏破裂术后死于多脏器功能衰竭 ,1例肺静脉破裂死于术中外 ,其余均治愈出院 ,无 1例出现高热、寒战及其他并发症。结论 :经胸管回收自体血 ,经处理后回输抢救胸外伤失血性休克安全有效 ,简便快速 ,节省血源 ,减少开支 ,减少异体输血并发症。
Objective:To evaluate the safety and feasibility of autotransfusion of blood collected by the chcst tube in the treatment of hemorrhagic shock due to chest trauma Methods:Between January 1996 and July 2000,46 patients with hemorrhagic shock due to chest trauma were treated with autotransfusion of blood collected by the chest tube Patients were given broad spectrum antibiotics during the operation and the routine antibiotics post operation Results:By using blood collected by the chest tube,autotransfusions were performed on 22 patients with an amount of 800~1 500 ml,17 patients with 1 501~2 000 ml,and 7 patients with >2 000 ml All patients recovered from the trauma and were discharged from hospital except 2 patients,one patient with rupture of heart died of multi organ faiure post operation and one patient died of rupture of pulmonary vein during the operation None of patients developed fever,chills or other complications Conclusions:Autotransfusion of processed blood collected by the chest tube is a safe effective treatment for hemorrhage shock due to chest trauma It provides advantages of simple,immediately available,blood soures,and reduces cost It can also avoid complications associated with allogeneic blood transfusion
