摘要 目的 评价CT仿真支气管内镜 (CTVB)成像方法及临床应用。材料与方法 对 5 2例经纤维支气管镜和病理证实的患者进行螺旋CT扫描 ,其中中心型肺癌 2 7例 ,气管癌 2例 ,肺癌术后 4例 ,支气管憩室 1例 ,正常支气管 18例。原始图像预处理后 ,传至计算机工作站 ,CTVB用“导航”软件进行观察。结果 CTVB逼真地显示气管、支气管腔、软骨环、隆突、左右主支气管至 5级支气管的开口 ,酷似纤维支气管镜所见。 2 7例中心型肺癌中 ,肿瘤呈肿块状或结节状 ,引起支气管狭窄 2 3处 ,闭塞 4处 ,肿瘤附近气管、支气管环模糊变平或消失 ,与纤维支气管镜所见一致。肺癌术后支气管残端呈光滑的盲端。支气管憩室在CTVB上呈局限性凹陷 ,在SSD上呈局限性突出。CTVB还能通过纤维支气管镜不能通过的狭窄处 ,或从远端向近端观察。结论 CTVB是一种新的无创性观察气管、支气管腔的方法 ,可部分代替纤维支气管镜检查。结合轴位和MPR图像 ,能同时显示管壁厚度和肿瘤向管腔外侵犯的范围。与纤维支气管镜相比 ,不能观察支气管粘膜病变和获取组织学标本是其主要限度。 Objective To evaluate the clinical application of CT virtual bronchoscopy (VB).Materials and Methods CTVB was performed in 52 patients with pathologically or bronchoscopically confirmed lung diseases, including central type lung cancer (n=27), tracheal carcinoma (n=2), post operated lung cancer (n=4), bronchial diverticulum (n=1) and normal bronchus (n=18). The preprocessed source images were input into the workstation and CTVB was performed with navigator smooth software (advantage windows 3.1 GE Medical Systems).Results CTVB could vividly reveal the tracheobronchial lumens, the cartilage rings, the carina and both right and left bronchi up to the fifth branching level orifices, quite mimicking fiberoptic bronchoscopy. Of 27 cases with central type lung cancer, bronchial stenosis was seen in 23 and bronchial occlusion in 4, while the bronchial rings close to the tumor displayed blurry, smooth or disappeared, in accordance with that seen on fiberoptic bronchoscopy. Post operated bronchial stump (n=4) showed a smooth blind end. Bronchial diverticulum exhibited a local concavity on CTVB or a local protrusion on SSD. CTVB could pass through stenotic bronchi to observe the occluded bronchi from the other side.Conclusion CTVB is a noninvasive technique and can partly replace fiberoptic bronchoscopy. Combined with axial and MPR imaging, CTVB can demonstrate the extraluminal extension of tumors, however, CTVB can neither detect mucosal abnormalities nor get histologic samples.
