摘要
目的 :探讨经阴道超声及彩色多普勒超声准确诊断卵巢恶性肿瘤的价值 ,鉴别卵巢良恶性肿瘤。方法 :47例有盆腔肿块的患者 ,术前应用阴道超声对肿块进行评分同时加用彩色多普勒超声检查获得 PI和 RI值。术后对照病理诊断了解诊断符合率。结果 :阴道超声对盆腔肿块评分 :B超积分 >9的 19例患者中 ,病理检查证实为恶性肿瘤 17例 ,诊断符合率为 89.5 %。B超积分≤ 9的 2 8例患者中 ,病理检查证实为良性肿瘤 2 6例 ,诊断符合率为 92 .9%。彩色多普勒超声测定肿块周边血流 :PI<1.0的例数 19例 ,病理检查证实为恶性肿瘤 18例 ,诊断符合率为 94.7%。RI<0 .5的例数 19例 ,病理检查证实为恶性肿瘤 19例 ,诊断符合率为 10 0 %。超声评分和血流 PI、RI值综合估计肿块良恶性程度 :B超积分 >9和 PI<1.0、RI<0 .5共 18例 ,病理检查均为恶性肿瘤 ,诊断符合率为 10 0 %。B超积分≤ 9和 PI≥ 1.0、RI≥ 0 .5共 2 7例 ,病理检查证实为良性肿瘤 2 6例 ,诊断符合率 96 .3%。结论 :B超积分 >9和 PI<1.0、RI<0 .5是卵巢恶性肿瘤较特异的超声表现 ,是鉴别卵巢良恶性肿瘤较好的手段。
Objectives:To study the value of transvaginal ultrasonography and color Doppler ultrasonography in the diagnosis of malignant ovarian tumors.To discriminate benign from malignant ovarian tumors.Methods:47 patients who had pelvic mass were studied by transvaginal ultrasonography to analyzed mass scoring,and by color Doppler ultrasonography to analyzed PI and RI of peripheral blood flow of mass.Results:Mass ultrasonography scoring:Scoring> 9,89.5 % masses were malignant tumors.Scoring≤9,92.9% masses were benign tumors.PI and RI of peripheral blood flow of masses detected by color Doppler ultrasonography:PI<1.0,94.7% patients were malignant tumors.RI<0.5,100% patients were malignant tumors.To differentiate benign and malignant tumor through ultrasonography scoring and PI and RI Overall:Mass scoring>9 and PI<1.0,RI<0.5 was found in 18 patients,their pathologic diagnosis was all malignant ovarian tumors,diagnosis match rate was 100%.Mass scoring≤9 and PI≥1.0,RI≥0.5 was found in 27 patients,26 patients pathologic diagnosis was benign ovarian tumors,diagnosis match rate was 96.3%.Conclusions:Ultrasonography scoring>9 and PI<1.0,RI<0.5 of mass peripheral blood flow were special ultrasonography features of malignant ovarian tumor,and were better methods to distinguish benign from malignant ovarian tumor.
