Resection of cerebral lesions by stereotactic microsurgical operation

摘要 目的 :探讨立体定向显微手术切除脑内病灶的可行性。方法 :应用F .L .FischerZD定向仪 ,CT定位 ,对 2 4例脑重要功能区及脑深部病灶进行立体定向开颅显微手术。病变性质 :胶质瘤 14例 ,脑囊虫 2例 ,转移瘤 2例 ,脑脓肿 1例 ,脑膜瘤 1例 ,动静脉畸形 (AVM) 1例 ,海绵状血管瘤 1例 ,异物 1例 ,炎性肉芽肿 1例。病灶直径为 1.5~ 3.0cm ,>3.5cm 2例 ,均在立体定向显微手术下行病灶全切除术。结果 :术后除 2例转移瘤最终死于全身广泛转移外 ,其余均完全恢复健康 ,无手术并发症及死亡。结论 :对于脑深部或重要功能区病灶 ,边界清楚 ,直径 <3 .0cm者 。 Objective:To study the fensibility of resecting cerebral lesions by stereotactic microsurgical operation.Methods:Fischer CT stereotactic apparatus was used.Of the 24 case of cerebral lesions,14 were located in the subcortex of movement or feeling area,3 cases in the subcortex of language center, 6 cases in the deep brain and 1 case in occipital lobe. Epilepsy, hemisphere, dysphasia headache and vomiting were the most common clinical manifestations. There were 14 cases of glioma, 2 cases of cerebral cysticercosis,2 cases of metastastic tumors, 1 cases of brain absess, meningioma, AVM, cavernous angioma, foregin objects and inflammatory granuloma respectively. The diameter of the lesions was 1.5~3.0 centimeters,with 2 cases more than 3.5 centimeters. Total resection was performed by steretastic microsurgical operation.Results:After the operations,the symptoms of all the patients gradually alleviated. Except that the 2 cases with metastastic tumor died of diffuse metastasis,the others recovered, whithout operative complication or death.Conclusion:The lesions located in the deep brain or important function area, if the lesion's diameter is less than 3 centimeters and the margin is clear, stereotastic microsurgical operation is more appropriate.
