摘要
目的探讨卡介菌多糖核酸联合特异性脱敏治疗对特应性皮炎患者IL-2与IL-4表达的影响。方法选择35例特应性皮炎患者作为治疗组,同时选择32名本单位健康职工作为正常对照组。治疗组患者应用卡介菌多糖核酸治疗共半年,同时采用阿罗格行脱敏治疗并持续至1年,采用放射免疫法测定对照组人群和治疗前后患者血清总IgE,IL-2和IL-4水平。结果特应性皮炎患者治疗前血清IL-4水平(5.11±1.47)μg/L和IgE水平(8.93±3.15)U/mL明显高于正常对照组(3.32±1.08)μg/L和(1.66±1.56)U/mL(P<0.01),治疗前患者IL-2水平(2.16±0.97)μg/L明显低于对照组(5.60±1.49)μg/L,差异有统计学意义(P<0.01);治疗1年后血清IL-2水平(4.51±1.76)μg/L较治疗前显著升高,而治疗后IL-4和IgE水平(3.32±1.08)μg/L和(4.62±2.15)U/mL较之治疗前明显降低,差异均有统计学意义(P<0.01)。相关性分析表明,血清IL-2水平与SCORAD得分呈显著负相关(P<0.01);而血清IL-4水平与SCORAD得分呈显著正相关(P<0.01)。结论卡介菌多糖核酸联合特异性脱敏可通过调控Th1/Th2细胞因子之间的平衡用以治疗特应性皮炎患者。
Objective To assess the effect of the combined treatment with bacille calmette-guerin polysaccharide nucleic acid (BCG-PSN) and specific hyposensitization on the serum level of IL-2 and IL-4 in atopic dermatitis patients. Methods A total of 35 patients with atopic dermatitis were selected as the therapy group, and the control group was composed of 32 healthy people currently working in our hospital. The patients were treated with BCG-PSN for half a year, combined with the hyposensitization treatment course with an unmodified semi-depot preparation ( Novo-Helisen Depot, NHD) for one year, Serum levels of IgE, IL-2 and IL-4 were measured by specific radioimmunoassay method before and/or after the treatment. Results The serum levels of IL-4 (5.11 ± 1.47)μg/L and IgE ( 8.93 ± 3.15 ) U/mL in the patients before the treatments were higher than those [ (3. 32 ± 1.08) μg/L and ( 1.66 ± 1.56) U/mL] in the people of control group whereas an obvious decrease of IL-2 level(2. 16 ±0. 97)iJ, g/L was found in the patients before the treatments compared with that(5.60 ± 1.49)μg/L in the people of control group (P 〈0. 01 ). An increased level of IL-2(4. 51± 1.76)μg/L, a de- creased level of IL-4( 3.32 ± 1.08 ) μg/L, and a decreased level of IgE(4. 62 ± 2. 15 )U/mL after the one-year treatment compared with those correspondings before the treatment were also found (P 〈 0. 01 ). Linear correlation analysis demonstrated that a significant negative correlation between serum levels of IL-2 and SCORAD scores( P 〈 0. 01 )and a significant positive correlation between serum levels of IL-4 and SCORAD scores( P 〈 0. 01 ). Conclusion Combination of BCG-PSN and specific hyposensitization can be considered to treat atopic dermatitis through the ability to regulate the balance of Thl/Th2.
