幼猪脊柱侧凸模型的弯型演变及其意义

Evolution of the curve pattern in a porcine scoliosis model and its significance in preclinical spinal implant testing

摘要 [目的]探讨幼猪后路不对称性栓系建立的脊柱侧凸模型中弯型演变的特点及其意义。[方法]选择8只雌性约克夏幼猪(6周龄,6~8kg),采用分段小切口的方式,在T5、T6及L2、L3左侧置入椎弓根螺钉固定,同侧竖脊肌旁切口暴露并绑扎第10-13肋骨,适当加压行钢缆栓系,使脊柱凸向右侧,建立脊柱侧凸模型。术后每4周进行脊柱x线检查。建模8周后,将后路内固定取出,不做任何治疗。观察记录冠状面及矢状面弯型特点,冠状面测量侧凸主弯Cobb角,记录顶椎位置,矢状面形态测量前凸及后凸Cobb角。[结果]1只因感染导致内固定松动,栓系失败。其余7只幼猪成功建立了脊柱侧凸模型,表现为进展性结构性脊柱侧凸,术后即刻Cobb角平均为34.5°,经过8周栓系,进展为59.3°。所有幼猪的侧凸均表现出显著的顶椎偏移中线以及顶椎旋转等类似于人类脊柱侧凸的特征。冠状面上,侧凸主弯的顶椎位置随栓系时间发展,逐渐移行至胸椎下段,矢状面上,由术后即刻的均匀的生理性后凸发展为胸椎前凸、胸腰段后凸的特征。[结论]幼猪中脊柱侧凸模型,在栓系过程中,会发生冠状面顶椎位置的改变和矢状面弯型的改变,在进行矫形技术的应用中应予注意。 [ Objective ] To investigate the evolution of the curve pattern with a progressive scoliosis model in immature pigs and its significance in preclinical spinal implant testing. [ Methods] Eight female pigs (age: 6 weeks, weight: 6 - 8 kg) were instrumented and tethered through three separate incisions. Pedicle screws were inserted in the left sides of Ts.6 and L2.3. Ipsilateral ribcage tethering of the left - sided T10- 13 ribs was also performed through a left - sided paramedian incision. Through mild intraoperative tensioning and repositioning of the animal with a right coronal curvature, a mild deformity was achieved. The pro- gression of the spinal deformity was documented through serial radiographs performed at 4 - week intervals. After 8 weeks, the posterior instrumentations were removed. [Results] One pig was eliminated from this study as it developed deep infection. Of the seven remaining for analysis, all developed a progressive, structural scoliosis with convexity to the right side. The mean initial Cobb angle of the scoliosis after posterior asymmetric tethering was 34.5° and progressed to 59. 3° after the 8 - week tethering period. All curves demonstrated characteristic radiographic features, such as remarkable apical vertebral translation and apical vertebral rotation. On the coronal plane, the apex location changed from the mid - thoracic to the lower thoracic region. On the sagittal plane, the curve pattern changed from a smooth kyphosis postoperatively, to a thoracic lordosis then a thoracolumbar kyphosis. [Conclusion] In the porcine scoliosis model, the apex and radiographic features of the sagittal plane may change during the tethering period. All of these should be taken into account when testing new spinal implants pre - clinically.
