原发性高血压患者瞬时波强技术及脉象同步检测的临床意义探讨 被引量:5

摘要 目的:探讨同步应用瞬时波强(wave intensity,WI)技术及脉象仪评估原发性高血压的价值及二者的相关性。方法:对35例原发性高血压患者(高血压组)与33例健康人(对照组)行同步颈动脉WI及桡动脉脉象检测,应用ALOKA公司Prosoundα7彩色多普勒超声诊断仪行颈动脉WI检测,应用DDMX-100脉象仪行桡动脉脉象参数检测,分析并比较2组WI参数及脉象参数。结果:高血压组WI参数中W1、NA较对照组增高,W1-W2较对照组降低(P<0.05);脉象参数中,h1、h3、h4、h3/h1、w1/t、w2较对照组增高,t4较对照组降低(P<0.05)。对照组W1与h1呈正相关(r=0.339,P=0.005)、W1-W2与t4呈正相关(r=0.680,P<0.001),NA与h5/h1呈负相关(r=-0.256,P=0.038);高血压组W1与h1呈正相关(r=0.351,P=0.003),W1-W2与t4呈正相关(r=0.509,P<0.001),NA与h4/h1呈正相关(r=0.272,P=0.023)。结论:WI与脉象参数间有相关性,WI和脉象同步检测为临床综合评估原发性高血压提供了新方法。 Objective: To explore the value of synchronously applying instantaneous wave intensity technique and pulsography in patients with primary hypertension and to investigate the correlation of the indexes of WI and parameters of pulse presentations. Methods: 35 primary hypertension patients (hypertension group) and 33 healthy people (control group) were included, examined by synchronously applying instantaneous wave intensity technique and pulsography. The indexes of WI of carotid artery were detected with Prosound a7 color Doppler ultrasonography made in ALOKA company. And the parameters of pulse presentations of radial artery were detected with DDMX 100 pulsography. All the parameters were researched and compared. Results: The indexes of WI,Wl and NA in hypertension group were significantly higher than that in control group;W1-W2 in hyperten sion group were significantly lower than that in control group (P〈0.05). In the parameters of pulse presentations, h1 , h3, h4 , h3/h1 , w1/t and w2 in hypertension group were significantly higher than that in control group;t4 in hypertension group were significantly lower than that in control group (P〈0.05). In control group, there were positive correlations between Wl and hi (r= 0. 339,P=0. 005),between Wl-W2 and t4 (r=0. 680,P〈0. 001),there were negative correlations between NA and h5/h1 (r= - 0.256, P = 0. 038). In hypertension group, there were positive correlations between W1 and h1 (r= 0.351, P = 0. 003), between W1 -W2 and t4 (r= 0. 509, P〈0.001 ), between NA and h4/h1 ( r = 0.272, P = 0. 023). Conclusion: There are significant correlations between the indexes of WI and parameters of pulse presentations. Synchronously applying instantaneous wave intensity technique and pulsography provides a new way to evaluate primary hypertension.
