摘要
目的：探讨运用医院质控系统结合三级质量控制在初产妇导乐分娩中的应用效果。方法选取我院2012年10月至2013年2月收治的100例初产妇按照护理管理方法的不同分为观察组和对照组，每组50例，对照组采用传统护理模式，观察组结合我院质控系统在PDCA循环三级质量控制管理下实施导乐分娩，比较两组产妇的产程进展情况、分娩结局、新生儿结局以及护理满意度。结果观察组的第一产程时间、第二产程时间、总产程时间显著短于对照组，差异有统计学意义（ P＜0.05）；观察组的阴道分娩率显著高于对照组，剖宫产率显著低于对照组，产后出血量显著少于对照组，差异有统计学意义（ P＜0.05）；观察组新生儿窒息率显著低于对照组，窒息评分显著高于对照组，差异有统计学意义（ P＜0.05）；观察组在总体服务、环境舒适、健康宣教等方面的护理满意度均高于对照组，差异有统计学意义（ P＜0.05）。结论运用医疗质控管理系统结合PDCA循环三级质量控制能够有效提高导乐分娩的质量，促进产妇的产程进展，改善分娩结局。
Objective To explore the application effect of three levels quality control in primipara with Doula delivery .Methods Totaled of 100 cases of primipara were divided into the observation group and the control group according to different nursing management , 50 cases in each group .The control group was used traditional nursing mode , and the observation group was given Doula delivery in three cy-cles of PDCA quality control management .Progress of birth , the outcome of childbirth , newborn outcome and nursing satisfaction were com-pared between the two groups .Results The time of the first stage , the second stage of labor time , total labor time in the observation group were significantly shorter than those in the control group , the vaginal delivery rate was significantly higher than that in control group , the a-mount of postpartum hemorrhage was significantly less than that in the control group with statistical significance （ P〈0.05 ） .The rate of neo-natal asphyxia in the observation group was significantly lower than that in the control group , the scores of asphyxia were significantly higher than those in the control group with statistical significance （ P〈0.05 ） .Nursing satisfaction in the observation group was higher than that in the control group （ P〈0.05 ） .Conclusions PDCA three quality control can effectively improve the quality of Doula delivery and promote the progress of the production process of maternal , improve birth outcomes .
