摘要
目的观察在常规药物治疗基础上辅以运动康复干预对冠心病合并2型糖尿病患者糖脂代谢及生活质量的影响。方法采用随机数字表法将76例冠心病合并2型糖尿病患者分为运动组(37例)及对照组(39例)。2组患者均给予常规药物(包括阿司匹林、阿托伐他汀、美托洛尔、二甲双胍等)治疗,运动组在此基础上辅以中等强度有氧运动。于入选时、治疗3个月后观察2组患者空腹血糖、糖化血红蛋白、血脂水平及生活质量变化情况。结果2组患者分别经3个月治疗后,发现其空腹血糖、糖化血红蛋白、总胆固醇、低密度脂蛋白及生活质量评分均较入选时明显改善;并且以运动组患者空腹血糖[(5.75±1.13)mmol/L]、糖化血红蛋白[(6.29±0.87)%]、总胆固醇[(3.77±0.66)mmol/L]、低密度脂蛋白[(2.02±0.50)mmol/L]及生活质量评分[其生理功能为(68.4±14.9)分,生理职能为(50.4±18)分,躯体疼痛为(57.6±20.4)分,总体健康为(53.9±17.3)分,生命活力为(57.3±21.7)分,社会功能为(66.9±20.0)分,情感职能为(69.8±22.5)分,精神健康为(80.0±18.5)分]的改善幅度较显著,与对照组间差异均具有统计学意义(P〈0.05)。结论在常规药物治疗基础上辅以康复运动干预,能进一步改善冠心病合并2型糖尿病患者糖脂代谢功能及生活质量,表明该联合疗法值得临床推广、应用。
Objective To explore the effects of rehabilitation exercise on glucose and lipid metabolism as well as on the quality of life of patients with coronary heart disease (CHD) and type 2 diabetes receiving conventional drug treatment. Methods Seventy-six hospitalized patients with both CHD and type 2 diabetes were randomly divided into an exercise group (37 cases) and a control group (39 cases). Both groups were given routine drug treatment (including aspirin, atorvastatin, metoprolol, metformin, etc. ). The exercise group started with moderate intensity aerobic exercise; the patients in the control group were just given routine care. After 3 months, fasting plasma glucose, glycosylated hemoglobin, blood lipids and any changes in the quality of life of the 2 groups were measured. Results Fasting plasma glucose, glycosylated hemoglobin, total cholesterol and low density lipoprotein cholesterol (LDL-c) levels decreased significantly in both groups, and the quality of life index in both two groups improved significantly compared with the baseline, but the improvements in the exercise group were significantly greater. Conclusion As a supplement to routine drug therapy, rehabilitation exercise can further improve glucose and lipid metabolism and the quality of life of patients with CHD and type 2 diabetes.
