摘要
目的:评价血清同型半胱氨酸(Hcy)和基质金属蛋白酶-2(MMP-2)检测在糖尿病肾病中诊断中的价值。方法:将91例糖尿病肾病患者随机分为无白蛋白尿组,微量白蛋白尿组和临床白蛋白尿组,另取同期门诊的健康体检者35例作为对照组,比较4组间Hcy和MMP-2水平的变化,应用Pearson's相关性分析和ROC曲线分析对各指标进行评价。结果:糖尿病肾病组患者Hcy水平明显升高,MMP-2水平明显降低,与对照组相比差异有统计学意义(P<0.05),且微量白蛋白尿组和临床白蛋白尿组与无蛋白尿组差异存在统计学意义(P<0.05);糖尿病患者的Hcy与蛋白尿排泄率呈正相关(r=0.57,P=0.024),MMP-2与蛋白尿排泄率呈负相关(r=-0.68,P=0.017);Hcy和MMP-2联合诊断的ROC曲线AUC面积大于各指标的单独诊断。结论:Hcy和MMP-2水平的变化与糖尿病肾病患者蛋白尿的排泄量有一定联系,可作为DN的早期监控和病变程度评估指标。
Objective:To evaluate homocysteine(Hcy) and matrix metalloproteinase-2(MMP-2) of early diagnosis in patients with diabetic nephropathy.Methods:91 patients were divided into without albuminuria group, microalbuminuria group and clinical albuminuria group, select health physical examination patient of 35 cases as control group, Hcy and MMP-2 were measured and compared with control group, evaluate the various indicators by Pearson's correlation analysis and ROC curve analysis.Results:Level of Hcy in patients with diabetic nephropathy group were increased significantly, and MMP-2 were decreased, compared with the control group significantly(P〈0.05), microalbuminuria group and clinical albuminuria group has highly statistically significant(P〈0.05)compared with control group. In DM patients, Hcy was positively correlation with UAER(r=0.57,P=0.024),but the MMP-2 had negative correlation with UAER (r=-0.68, P=0.017) . ROC areas of combined examination are more than each one.Conclusion:The change of Hcy and MMP-2 have certain contact on proteinuria emissions in patients with diabetic nephropathy, and which mightbe play a key role in the pathogenesis and progression of diabetic nephropathy patients.
