摘要
通过分析目前国内外延续护理及双向转诊开展情况的研究结果,针对《卫生事业发展"十二五"规划》中加强延续护理体系构建的要求,结合我院双向转诊试点工作的实践,分析依托双向转诊平台建立延续护理的可行性,提出通过开展县-社区医疗联动模式,实施延续护理管理干预,能在提高转诊率的同时,推进延续护理管理体系的建立。
Based on the results of the current international continuing care and two-way referral practice, according to the requirement of our country with the nursing program, considering pilot work of two-way referral practice in our hospital, we analyze the feasibility of the proposed continuing care based on two-way referral platform and establish the continuing care management intervention through the county-community medical linkage mode, which might improve the referral rate and promote the establishment of continuing care management system.
