摘要
目的探讨血清畸胎瘤细胞衍生生长因子(PCDGF)、血管内皮生长因子(VEGF)及细胞质胸苷激酶1(TK1)与肺癌临床病理参数的相关性,判断其在肺癌联合诊断中的价值。方法用ELISA法检测123例肺癌患者、60例肺良性疾病患者和61例体检健康者血清PCDGF、VEGF的含量;酶免疫点印迹化学发光法检测标本中TK1的含量。结果肺癌组血清PCDGF、VEGF、TK1含量分别为18.27(6.24,58.86)ng/mL、1072.73(272.42,2 686.46)ng/mL和1.59(0.21,8.21)pmol/L,均显著高于肺良性疾病组及健康对照组(Z分别为81.349、85.769和18.818,P均<0.05)。血清PCDGF含量与肿瘤分期、淋巴结转移均有统计学意义(Z分别为8.063、2.488,P均<0.05)。VEGF含量对区分不同组织学类型肺癌具有统计学意义(Z=6.634,P<0.05)。经直线相关性分析表明,血清PCDGF与VEGF含量间无相关性(r=0.017,P=0.786)。PCDGF诊断肺癌的敏感性为83.7%,三项联合检测的敏感性最高为95.9%,TK1的特异性最高为90.7%,而三项指标的准确性均较低。结论PCDGF、VEGF、TK1含量在肺癌患者血清中显著增高。PCDGF与肺癌分期、淋巴结转移相关,有望作为肺癌诊断及恶性程度评价的潜在血清标志物。
Objective To explore the correlations of serum PC cell-derived growth factor(PCDGF), vascular endothelial growth factor (VEGF) and thymidine kinase 1 ( TK1 ) levels with clinical pathological parameters in lung cancer, and their combined value in the diagnosis of lung cancer. Methods Serum PCDGF and VEGF levels were detected by ELISA, and serum TK1 level by chemiluminescence enzyme immunoassay in 123 patients with lung cancer, 60 patients with benign lung disease and 61 healthy controls. Results The levels of serum PCDGF, VEGF and TK1 in the patients with lung cancer were 18.27 (6.24, 58.86) ng/mL, 1 072.73 (272.42, 2 686.46) ng/mL and 1.59(0.21, 8.21 ) pmol/L, respectively, and were significantly higher than those with benign lung disease and the healthy controls (Z = 81. 349, 85. 769 and 18. 818, respectively, P 〈 0.05 ). Serum PCDGH level was significantly related to tumor staging and lymph node metastasis ( Z = 8. 063 and 2. 488, P 〈 0.05 ), while VEGF level related to different histological types of lung cancer(Z = 6. 634, P 〈 0.05 ). There was no any statistical correlation between serum PCDGF and VEGF levels( r = 0. 017, P = 0. 786). The sensitivity of PCDGF was 83.7% for the diagnosis of lung cancer, and when PCDGF, VEGF mad TK1 were combined, the sensitivity increased to 95.9%. The specificity of TK1 was 90.7% , which was the highest among the three markers. The accuracy of all the three markers was low. Conclusion The levels of serum PCDGF, VEGF and TK1 significantly increased in the patients with lung cancer. PCDGF was associated with lung cancer staging and lymph node metastasis, which may be used as a potential marker for the diagnosis of lung cancer and the malignant evaluation.
