特定浓度高渗钠盐复苏液对严重烫伤大鼠早期肺损伤的影响 被引量:1

Effects of Na+hypertonic saline resuscitation on pulmonary injury of severly scalded rats in the early stage

摘要 目的探讨特定浓度(200 mmol/L)高渗钠盐复苏液对严重烫伤大鼠早期肺损伤的影响。方法 56只SD大鼠随机分成烫伤组(48只)和对照组(8只),其中烫伤组又分为乳酸钠林格液(LR)组(n=24)和高渗钠盐液(HS)组(n=24)。所有大鼠全部颈静脉置管,对照组大鼠不烫伤不补液,对LR组和HS组大鼠进行烫伤后复苏治疗,分别检测各组肺组织的髓过氧化物酶(MPO)与丙二醛(MDA)的含量,将所得数据进行统计学分析;采用免疫组织化学方法测定肺组织p38MAPK的活化程度,所得图片进行比较。结果 HS组复苏后肺组织的MPO含量在注射补液2、8、24 h分别为(1.48±0.45)、(1.52±0.31)、(1.46±0.15)U/g与补液2、8、24 h MDA含量(0.80±0.09)、(0.85±0.08)、(0.80±0.12)mmol/mg明显低于对应观测时间点的LR组复苏后肺组织的MPO含量(1.91±0.32)、(1.91±0.32)、(1.79±0.22)U/g与MDA含量(1.06±0.08)、(0.85±0.08)、(0.95±0.06)mmol/mg,差异有统计学意义(P<0.05);与对照组的MPO含量(1.49±0.22)U/g与MDA含量(0.78±0.14)mmol/mg比较,差异无统计学意义(P>0.05)。肺组织的p38MAPK的免疫组化活性检测显示:HS组复苏后2、8、24 h的肺组织p38MAPK的表达与LR组比较明显减少,与对照组比较增加不明显,LR组与对照组比较明显增加。结论严重烫伤大鼠早期使用200 mmol/L高渗盐液进行液体复苏与乳酸钠林格液比较,通过减少肺组织的氧化应激反应与信号转导通路的活化减轻肺损伤。 Objective To explore the effect of 200 mmol/L Na+hypertonic saline resuscitation on pulmonary injury of severely scalded rats in the early stage. Methods Fifty-six SD rats were randomly divided into scald group and control group,and the scald group was divided into sodium lactate Ringer' s solution( LR) group and hypertonic sodium solution( HS) group, each group of 24. Neck vein catheterization was used on all rats,and scald resuscitation therapy was adopted in the rats of LR group and HS group. Lung tissue myeloperoxidase(MPO) and lung tissue malondialdehyde(MDA) were examined in three groups. The data drawn from the test were statistically analyzed. The immunohistochemical method was used to determination p38MAPK activation level of lung tissue in the control group and after fluid resuscitation in the group LR and group HS. The pictures were compared. Results The pulmonary tissue MPO levels of the group HS at 2 h(1. 48 ± 0. 45),8 h(1. 52 ± 0. 31) and 24 h(1. 46 ± 0. 15) U /g after resuscitation were significantly lower than those of the group LR at 2 h(1. 91 ± 0. 32),8 h(1. 91 ±0. 32) and 24 h(1. 79 ± 0. 22) U /g respectively,and the pulmonary tissue MDA levels of the group HS at2 h(0. 80 ± 0. 09),8 h(0. 85 ± 0. 08) and 24 h(0. 80 ± 0. 12) mmol /mg after resuscitation were significantly lower than those of the group LR at 2 h(1. 06 ± 0. 08),8 h(0. 85 ± 0. 08) and 24 h(0. 95 ±0. 06) mmol /mg respectively,the difference was statistically significant( P〈0. 05 or P〈0. 01). But compared with the control group on MPO level( 1. 49 ± 0. 22) U /g and MDA level( 0. 78 ±0. 14) mmol /mg,the pulmonary tissue MPO and MDA levels of the group HS had no significant difference(P〉0.05). The p38MAPK activity contents of lung tissue by immunohistochemical assay in the group HS at 2 h,8 h,24 h after fluid resuscitation were significantly expressed in comparison with the group LR,and had no obviously difference with the control group. The p38MAPK activity contents of lung tissue in the group LR at 2 h,8 h and 24 h after resuscitation increased significantly expression compared with the control group. Conclusions Compared with LR,supplementing HS can attenuate oxidative stress and p38MAPK activity contents of lung tissue significantly,which ameliorate pulmonary injury of severely scalded rats in the early stage.
