皮肤消毒范围对预防中心静脉导管血流感染的影响 被引量:10

INFLUENCE OF SKIN DISINFECTION RANGE FOR PREVENTING BLOODSTREAM INFECTION OF THE CENTRAL VENOUS CATHETER

摘要 目的研究皮肤消毒范围对预防中心静脉导管血流感染效果的影响。方法采用细菌培养法和临床观察,对扩大中心静脉置管穿刺部位皮肤消毒范围及其置管期间护理消毒效果进行观察。结果将中心静脉置管部位皮肤消毒范围由5cm×5cm扩大到10cm×12cm,试验组患者中心静脉置管穿刺口及其周围皮肤和导管尖端细菌培养阳性率分别为20.6%、24.7%和16.9%。消毒范围仍为5cm×5cm的对照组患者相同部位细菌培养阳性率分别为28.8%、32.5%和26.9%。试验组患者导管相关性血流感染发生率为1.56%,对照组患者为4.38%。结论中心静脉置管部位皮肤扩大消毒范围对于有效防控中心静脉导管血流感染的发生具有明显效果,值得推广使用。 Objective To study the effect of skin disinfection range on preventing bloodstream infection of the central venous catheter. Methods The disinfection effect of the enlarging skin disinfection range of puncture site in the central venous catheterization and during its catheter care was evaluated by the bacterial culture and clinical observations. Results In the patients of test group,the skin disinfection range of the central venous catheterization site from 5cm × 5cm extended to 10 × 12cm. The positive rate of bacterial culture in the puncture point,the around of skin puncture point and the tip of catheter were 20. 6%,24. 7% and 16. 9% respectively. In the patients of control group,the skin disinfection range of the central venous catheterization site remained 5cm × 5cm,the positive rate of bacterial culture in the above parts were 28. 8%,32. 5% and 26. 9% respectively. The catheter related bloodstream infection rate was 1. 56% in test group, 4. 38% in control group. Conclusion The expansion of skin disinfection scope in the central venous catheterization site has the marked effect of preventing the bloodstream infections of the central venous catheter,and it is worth promoting the use.
