摘要
目的探讨3种不同的穿刺方法对血透动静脉内瘘保护作用情况。方法分析2011年6月—2013年6月收治的透析患者60例临床资料,依据穿刺方法不同进行随机分组,绳梯法组20例,扣眼法组20例,区域法组20例。结果扣眼法组在引起内瘘血管瘤0、血管狭窄1例(5%)、出血1例(5%)、感染0发生率均明显低于绳梯法穿刺组内瘘血管瘤1例(5%)、血管狭窄2例(10%)、出血2例(10%)、感染1例(5%)发生率,χ2值=5.13、50.00、1.80、1.80、1.80、5.56、5.13、5.13,均明显低于区域法穿刺组内瘘血管瘤8例(40%)、血管狭窄2例(10%)、出血3例(15%)、感染1例(5%)发生率,50.00、1.80、5.56、5.13,P<0.05,差异有统计学意义。结论扣眼法穿刺可以降低内瘘血管瘤、血管狭窄、出血、感染发生率,对内瘘形成有效的保护,提高透析治疗效果,保证良好的预后,值得临床推广应用。
Objective To investigate the protective effect of three different puncture methods on internal arteriovenous fistula. Methods The clinical data of 60 cases of dialysis patients treated from June, 2011 to June, 2013 were analyzed. The patients were divided into ladder group of 20 cases, eyelets group of 20 cases and regional group of 20 cases according to different puncture methods. Results Of the eyelets group, there was 0 case of fistula hemangioma, 1 case of vascular stenosis （5%）, 1 case of bleed-ing（5%）, 0 case of infection, the incidence was significantly lower than that of the ladder group, respectively, of which there was 1 case of fistula hemangioma（5%）, 2 cases of vascular stenosis（10%）, 2 cases of bleeding （10%）, 1 case of infection（5%）, the χ2 value was 5.13, 50.00, 1.80, 1.80, 1.80, 5.56, 5.13, 5.13, and the regional group had the highest incidence of adverse reactions, of which, there were 8 case of fistula hemangioma（40%）, 2 cases of vascular stenosis （10%）, 3 cases of bleeding （15%）, 1 case of in-fection （5%）,χ2 value was 50.00, 1.80, 5.56, 5.13,P〈0.05,the differences were statistically significant. Conclusion Eyelets dialysis can reduce the incidence of internal arteriovenous fistula, vascular stenosis, bleeding and infection, and protect the fistula forma-tion effectively, improve the dialysis treatment effect, ensure a good prognosis and which is worthy of clinical promotion.
