3D-DSA MIP在颅内动脉瘤术后随访中应用初探 被引量:5

摘要 目的探讨3D-DSA最大密度投影(MIP)重组技术在颅内动脉瘤术后随访中的应用价值。方法搜集广东省人民医院2008年7月至2013年4月进行颅内动脉瘤栓塞或夹闭术后,行3月以上3D-DSA随访的病例203例,比较分析3D-DSA MIP与2D-DSA的随访复查结果和3D-DSA 3种后处理技术的差异。结果 203例患者共221个动脉瘤。3D-DSA随访复查,发现25例复发。而行2D-DSA成像时,漏诊率达16%(4/25)。3种3D-DSA后处理技术均发现复发,但光透技术(TR)和容积再现技术(VRT)未能显示瘤体内的弹簧圈填塞情况。结论 3DDSA-MIP重组技术能够提高颅内动脉瘤复发的正确诊断率,在颅内动脉瘤DSA随访中具有重要应用价值。 Objective To investigate the clinical value of 3D DSA maximum intensity projection reconstruction tech- nique for intracranial aneurysms follow up. Methods The data of 153 cases with intracranial aneurysms treated by coiling or clipping through July 2008 to April 2013 in Guangdong General Hospital were analyzed. The differences between 3D DSA and 2D DSA in follow up were compared. Results 221 aneurysms were found in 203 cases. For 3 - month follow up, aneurysms recurred in 25 cases. Among the 25 recurred cases, 4 cases were found on 3D DSA MIP images, and were missed by 2D DSA. All the 3 kinds of reconstruction techniques ( MIP, VRT,TR) of 3D-DSA could find the aneurysms, but TR and VRT could not show the coils clearly. Conclusion 3D DSA MIP reconstruction technique may improve the accuracy rate of recurred intracranial aneurysms and have an important role during the DSA follow up of intracranial aneurysms.
