摘要 目的:观察超声诊断不同病因所致浅表淋巴结肿大的临床价值,为今后临床诊治提供参考和借鉴。方法:本研究随机选取我院2012年5月-2013年11月期间收治的浅表淋巴结肿大患者47例,根据患者的不同致病原因分为5组,A组16例为慢性淋巴结炎,B组4例为淋巴结结核,C组7例为急性淋巴结炎,D组5例为淋巴瘤,E组15例为转移性恶性肿瘤,所有患者均进行超声检查,观察5组患者的超声表现。结果:5组患者在淋巴结数目、形态、包膜状态、融合情况等方面均有所不同;5组患者在淋巴结大小以及彩色多普勒血流显像特点等方面均有所不同,其中A组VS(Ⅲ率)明显高于E组(χ2=42.5926,P=0.0000),A组RI、PI明显低于E组(P<0.05),差异有统计学意义。结论:超声技术诊断不同病因所致的浅表淋巴结肿大能有效地检测出患者淋巴结的病变情况,为临床判断其病因提供可靠有效的依据,值得在临床实践中广泛的应用和推广。 Objective: This study observes the clinical value of high-frequency ultrasound in the diagnosis of superficial lymphadenopathy which is caused by different factors. It provides a reference for future clinical treatment. Methods: This study selects 47 patients with superficial lymphadenopathy randomly who were treated in our hospital from May 2012 to November 2013. According to the different causes, the patients are divided into 5 groups. Group A includes 16 chronic lymphadenitis cases. Group B includes 4 lymphoid tuberculosis cases. Group C includes 7 acute lymphadenitis cases. Group D includes 5 lymphoma cases. Group E includes 15 metastatic malignancy cases. All patients are examined by ultrasound. Then the study observes the ultrasonic manifestations of them. Results: Patients in the 5 groups are different in lymph nodes number, form, capsule, fusion. The 5 group patients are different in the size of lymph nodes and the characteristics of color doppler flow imaging. VS (Ⅲ rate) of group A is significantly higher than that of the group E (X^2=42.5926, P=0.0000).The RI and PI of group A are significantly lower than those of group E (P〈0.05). The differences are statistically significant. Conclusion: It can detect lymph nodes" lesions effectively when diagnosing superficial lymphadenopathy, which is caused by different factors, through ultrasound. When judging the cases, it provides reliable and effective basis. It is worthy to be applied and promoted widely in clinical practice.
