摘要
目的：观察头孢呋辛序贯疗法治疗社区获得性肺炎的临床疗效和安全性。方法：280例社区获得性肺炎患者按随机数字表法均分为常规疗法组和序贯疗法组。两组患者均给予退热祛痰和止咳平喘等常规治疗后，常规疗法组患者给予头孢呋辛1．5g加入5％葡萄糖注射液250ml中静脉滴注，tid；序贯疗法组患者给予头孢呋辛1．5g加入5％葡萄糖注射液250ml中静脉滴注，tid，3～5d后视病情改善情况给予头孢呋辛500mg，口服，bid。两组患者疗程均为5～10d。观察两组患者的临床疗效、临床各指标恢复情况、细菌转阴株数及不良反应发生情况。结果：两组患者总有效率、细菌总转阴株数比较，差异均无统计学意义（P〉0．05）；序贯疗法组患者平均住院时间显著短于常规疗法组，不良反应发生率显著低于常规疗法组，两组比较差异均有统计学意义（P〈0．05）；两组其余临床各指标恢复情况比较差异无统计学意义（P〉0．05）。结论：头孢呋辛序贯疗法治疗社区获得性肺炎疗效和安全性均较好。
OBJECTIVE： To observe clinical efficacy and safety of cefuroxime-sequentiak therapy in the treatment of communi- ty acquired pneumonia （CAP）. METHODS： 280 patients with CAP were randomly divided into routine treatment group and sequen- tial therapy group. Both groups were given routine treatment of removing fever, eliminating phlegm, relieving a cough, preventing asthma, etc. Routine treatment group was given cefuroxime 1.5 g added into Glucose injection 250 ml intravenously, rid; sequential therapy group was given cefuroxime 1.5 g added into Glucose injection 250 ml intravenously, rid, for 3-5 days, and then given cefuroxime axetil 500 mg orally, bid, according to the condition of disease. Treatment course of 2 groups lasted for 5--10 days. Clinical efficacy, clinical indicators, the number of bacterial negative conversion and ADR were observed in 2 groups. RESULTS： Total effective rates and total number of bacterial negative conversion had no statistical significance in 2 groups （P〉0.05）. Average hospitalization stay of sequential therapy group was significantly shorter than that of routine treatment group; the incidence of ADR was significantly lower than in routine treatment group; there were statistical significances （P〈0.05）. Other clinical indicators had no statistical significance between 2 groups （P〉0.05）. CONCLUSIONS： Cefuroxime-sequential therapy is effective and safe in the treatment of CAP.
