摘要
目的 探讨内源性洋地黄样物质 (EDL S)和心钠素 (ANP)与慢性肺心病患者的关系。方法 应用放射免疫法测定了 84例慢性肺心病患者、36例慢性阻塞性肺疾病 (COPD)患者和 2 2名健康人血清 EDL S和血浆 ANP浓度。结果 肺心病患者 EDL S、ANP均高于 COPD患者 (t分别为 5 .5 2 5 8、6 .9893,P均 <0 .0 0 1)和健康人 (t分别为 4.8898、6 .2 0 6 4,P均 <0 .0 0 1)。且 EDL S和ANP之间存在非常显著的正相关 (T=0 .476 8,P<0 .0 1)。肺心病患者 EDL S和 ANP浓度随心功能不全程度加重而升高 ,均呈显著正相关 (r分别为 0 .9397、0 .8886 ,P均 <0 .0 0 1) ,肺心病缓解后 EDL S、ANP浓度较急性发作期明显下降 (t分别为 2 .416 2、2 .7747,P均 <0 .0 5 )。结论 测定肺心病患者外周血 EDL S和 ANP浓度能估计患者心功能状态 ,并能监测病情变化。
Objective To explore the relationship between concentration of Endogenous Digoxin like Substance(EDLS),Atrial Natriuretic Peptide(ANP) and the patients with cor pumonale.Methods The concentrations of EDLS and ANP were determined in 84 patients with cor pumonale,36 patients with chronic obstructive pulmonary disease(COPD)and 22 healthy controls by RIA technique.Results The levels of EDLS and ANP in patients with cor pumonale were significantly higer than that in patients with COPD(P<0.001) and healthy controls(P<0.001).The rising of concentration of EDLS and ANP correlated positively with the severity of cor pumonale(r=0.4768,P<0.01) and the degree of cardiac failure(r=0.9397,r=0.8886,respectively,P<0.001)?The concentration of EDLS and ANP of cor pumonale decreased significantly after remission(P<0.05).Conclusions The concentrations of EDLS and ANP may be useful to monitor the cardiac function,to adjust treatment,and to evaluate its effect in cor pumonale.
