摘要
目的 探讨氯沙坦、福辛普利和硝苯地平控释剂对高血压病患者心脏结构、血流及功能的影响，并进行比较。方法 87例轻中度高血压病患者随机分为3组，分别应用氯沙坦50mg/d(L组，30例) 、福辛普利10mg/d(F组，28例) 和硝苯地平控释剂30mg/d(N组，29例)， 应用超声心动图(UCG)，观察比较治疗前、治疗4周末及16周末诊所血压（CBP），心脏功能及左室心肌肥厚的逆转情况。结果 高血压病各组不良反应发生率分别为L组18.2%、F组39.4%、N组43.8%。治疗4周后CBP均有显著性降低，三组间无显著性差异，以硝苯地平控释剂组作用显著。16周末各组总有效率均达85%以上。4周末各组左室舒张功能均显著改善(P<0.05～0.001)。16周末各组左室肥厚逆转，以福辛普利与氯沙坦组作用显著(P<0.05)。结论 氯沙坦、福辛普利和硝苯地平控释剂均能有效持续地降低血压，改善高血压病早期的左室舒张功能损害。
Purpose This study was aimed at investigating and contrasting the effects of Losartan, Fosinopril and Nifedipine GITS on cardiac structure and function in patients with essential hypertension (EH). Methods Eighty seven patients with mild to moderate EH were randomly divided into three groups and were treated with Nifedipine GITS 30mg/d ( n =29), Fosinopril 10mg/d ( n =28) or Losartan 50mg/d ( n =30) separately. Cardiac structure and function were measured with echocardiography before and after treatments of 4 and 16 weeks. Results The antihypertensive effects of three medicines had no statistical differences. Left ventricular diastolic function was improved at the end of 4 weeks ( P <0.05～0.001). Left ventricular hypertrophy was regressed after 16 weeks treatment. The regression in F group was the largest and that in N group the smallest ( P <0.05). Conclusion Three antihypertensive drugs all can significantly and safely decrease blood pressure, had strong effects on reversing ventricular hypertrophy, improve left ventricular diastolic function in early stage of hypertension.
