炎症因素及氧化应激与心房颤动心肌重构的相关性研究 被引量:29

摘要 目的探讨炎症因素及氧化应激与心房颤动(AF)心肌重构的相关性。方法采用免疫层析原理、循环酶法及库尔特原理分别对阵发性房颤组(50例)、持续性房颤组(58例)、永久性房颤组(40例)及正常对照组(56例)进行高敏C-反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)、白细胞及中性粒细胞水平检测;采用多普勒超声测量心腔结构和功能。结果在超声指标中,阵发性房颤组与对照组相比,左心房直径(LAD)有增大趋势,但差异无统计学意义,持续性房颤组和永久性房颤组左心房直径大于阵发性房颤组和对照组(P<0.05),且以永久性房颤组左心房直径增大明显;对右心房上下径(RAD)分析发现与LAD变化一致;与对照组比较,房颤组左室射血分数(LVEF)降低(P<0.05),左室舒张末期内径无统计学意义。在炎症指标中,房颤各组血清hs-CRP组、Hcy浓度明显高于对照组(P<0.05);持续性和永久性房颤组血清hs-CRP、Hcy浓度高于阵发性房颤组(P<0.05),持续性房颤和永久性房颤组血清CRP、Hcy浓度无明显差异。在白细胞和中性粒细胞计数上,各组间无统计学意义,但均高于对照组。结论炎症因素及氧化应激参与了心肌重构,并在心房颤动的发生、发展与维持过程中起了重要作用。
