摘要
目的:探讨保留脾血管的完全腹腔镜胰体尾切除术治疗胰体尾肿瘤的应用体会。方法:2013年6月我院行保留脾血管的完全腹腔镜胰体尾切除术治疗胰体尾神经内分泌肿瘤1例,结合复习相关文献。结果:手术在全腹腔镜下成功完成,手术时间150 min,术中出血120 mL。无胰瘘发生,术后5 d出院,随访至今未见肿瘤复发。结论:保留脾血管的腹腔镜保脾胰体尾切除术对选择性患者是安全可行的。它对术后快速康复、改善预后等方面具有重要的意义,可作为胰体尾良性和交界性肿瘤的首选术式。
Objective： To explore the application experience of laparoscopic splenic vessel-preserving distal pancreatectomy for tumors in the tail of the pancreas. Methods： On June, 2013, a case with the neuroendocrine tumor in the distal region of the pancreas was treated by purely laparoscopic splenic vessel-preserving distal pancreatectomy in our department, and we reviewed references concerned. Results： The operation was completed successfully using laparoscopic procedure, with the operation time of 150 minutes and the intraoperative blood loss of 120 mL. There was no pancreatic leak, and postoperative hospitalization was 5 days, so far no tumor recurrence was observed after the operation. Conclusions： laparoscopic splenic vessel-preserving distal pancreatectomy is safe and feasible for selective patients. It has important significance for rapid recovery and improving prognosis and so on, and it may be the first choice for the benign and borderline tumor in the distal region of the pancreas.
