摘要
目的探讨中西医结合治疗糖尿病周围神经病变的临床疗效。方法将2009年1月至2013年4月收治的86例糖尿病周围神经病变患者随机分为治疗组和对照组各43例。对照组在控制血糖的基础上给予口服甲钴胺治疗,治疗组在对照组治疗的基础上给予中药经典方剂黄芪桂枝五物汤联合针灸治疗,两组均连续治疗3周为1个疗程,观察比较两组治疗前后临床疗效、临床症候及神经体征评分、神经传导速率等。结果治疗组临床总有效率为93.02%(40/43),对照组为76.74%(33/43),两组比较,差异有统计学意义(P<0.05);治疗组治疗后临床症候及神经体征评分[(2.9±1.7)、(2.5±1.5)分]均低于对照组[(5.4±2.5)、(3.9±1.7)分],差异有统计学意义(P<0.05);治疗组治疗后腓总神经、正中神经传导速度明显优于对照组,差异有统计学意义(P<0.05)。结论糖尿病周围神经病变是糖尿病的常见并发症之一,临床应用中西医结合治疗能明显缓解患者症状和体征,提升神经传导速率,提高患者生活质量,值得在临床推广应用。
Objetive To investigate the clinical efficacy of combination therapy of traditional Chinese medicine （TCM） and Western medicine（WM） for diabetic peripheral neuropathy（DPN）. Methods Clinical data of 86 patients with DPN,who were received from January 2009 to April 2013 ,were collected and divided into treatment group and control group ,43 cases in each group. The control group was administrated with mecobalamin orally on the basis of glucose control ,while the treatment group was given Huangqi Guizhi Wuwu decoction combined with acupuncture therapy based on the therapy of control group. 3 weeks of continuous treatment in both groups was a course. The clinical efficacy ,nerve conduction velocity （NCV） and so on before and after treatment in both groups were observed and compared. Results The total effective rate was 93.02%（40/43）in the treat-ment group and 76.74%（33/43） in the control group with statistically significant difference（P〈0.05）. After treatment,the nervus peroneus communis NCV and median NCV in the treatment group were significantly superior to those in the control group （P〈0.05）. Conclusion DPN is one of common complications of diabetes. The clinical combination therapy of TCM and WM can al-leviate the symptoms and signs,improve nerve conduction velocity and increase the life quality of patients,so,it is worthy of popu-larization and promotion in clinic.
