摘要
目的研究Akin截骨联合第1跖骨基底楔形植骨治疗中重度拇外翻的疗效。方法2011年1月～2012年12月，采用Akin截骨联合第1跖骨基底楔形植骨治疗22例（32足）中重度足拇外翻；比较治疗前后拇外翻角（HAV）、第1、2跖骨间夹角（IMA）、美国足踝矫形学会拇外翻评分。结果手术前后IMA（分别为18．51°±4．03°，9．65°±3．79°）、HAV（分别为39．74°±5．94°，12．85°±3．84°）比较差异有统计学意义（P〈0．05），拇外翻评分优良率为93．75％。所有患者均顺利完成手术，切口全部一级愈合，未出现严重并发症。结论Akin截骨联合第1跖骨基底楔形植骨矫形效果好，治疗优良率高，适用于中重度拇外翻。
Objective To explore the effect of Akin osteotomy combined with first matatarsal basilar wedge bone graft in moderate and severe pollex valgus. Methods From Jan 2011 to Dec 2012, 22 cases （32 feet）of moderate and severe pollex valgus were treated with Akin osteotomy combined with first matatarsal basilar wedge bone graft. The improvement of pollex valgus angle and clinical symptom was observed. Results Postoperative IMA was （9.65 ± 3.79）° on average,improved greatly compared with preoperative IMA （18.51 ± 4.03 ）° （ P 〈 0.05 ）. Postoperative HAV was （ 12.85± 3.84 ）° on average, improved greatly compared with preoperative HAV （ 39. 74 ± 5.94）° （ p 〈 0.05 ）. The excellent and good rate was 93.75%. The operation was successfully performed in 22 cases and all patients were primary healing. Nine months follow - up showed that there was no case of severe complications. Conclusion Akin osteotomy combined with first matatarsal basilar wedge bone graft is a effective method to treat moderate and severe pollex valgus with high successful rate.
