摘要
目的分析鱼腥草注射液联合氟康唑静脉注射对普胸外科术后肺部真菌感染的疗效。方法选取重庆医科大学附属第一医院普胸术后肺部侵袭性真菌感染病例50例。随机分为对照组、治疗组。对照组采用标准剂量氟康唑静滴,治疗组采用标准剂量氟康唑联合鱼腥草注射液静滴。观察两组临床治疗效果,随访血常规、胸片及真菌培养和涂片。用药期间2组也密切监测肝肾功、电解质及临床不良反应。结果治疗组有效率高于对照组。治疗组在抗生素使用时间、辅助通气率、辅助通气时间、住ICU时间方面较对照组明显改善。治疗组死亡病例1例,对照组死亡病例4例。两组不良反应观察无明显差异(P>0.05)。结论鱼腥草注射液联合氟康唑静脉注射可用于普胸外科术后肺部侵袭性真菌感染治疗,不增加肝肾毒性及电解质紊乱的风险,并且可改善治疗的有效率,减少抗生素使用时间、辅助通气以及住ICU时间。
Objective： To investigate the clinical effects and safety of herba houttuyniae and fluconazol in invasive pulmonary fungous infection after general thoracic surgery and to find a new idea treating invasive pulmonary fungous infection after general thoracic surgery. Methods： 50 patients with invasive pulmonary fungous infection after general thoracic surgery were randomly divided into therapy group and control group.In control group ,the subjects were treated with fluconazole intravenous infusion,while in therapy group,subjects were not only treated with fluconazole intravenous infusion,but also with Herba houttuyniae 100 mL intravenous infusion once per day. Clinical effects were observed, and blood routine and chest X-ray were detected. Meanwhile,liver and renal function,blood electrolytes and other side effects were observed. Results： The basic clinical data was no statistical different between 2 groups. The clinical effective rates were 68% and 84% in control group and therapy group respectively. The time of using antibiotic,the rate of mechanical ventilation,mechanical ventilation time,ICU stay were improved in therapy group compared with control group（P〈0.05）. There was 1 patient died in therapy group and 4 patients died in control group. There were no statistical differences between the 2 groups in aspect of clini- cal side effects. Conclusion： Herba houttuyniae and fluconazol are effective and safe treating invasive pulmonary fungous infection after general thoracic surgery.
