摘要
目的:初步探讨活动期类风湿关节炎(RA)的中医证候分布特征。方法:采用SPSS(13.0),选择活动期RA患者126例,研究中医证候与病程,类风湿因子(RF)、C反应蛋白(CRP)、血沉(ESR)等实验室指标及晨僵等主要临床症状、体征,X线征象分期之间的相关性。结果:活动期RA患者证候分布比例差异较大,不同证候在病程、双手X线分期上存在差异。活动期RA中医证候在主要临床症状、体征与相关实验室指标上差异无显著性(P>0.05)。结论:"邪从热化"趋势是活动期RA病机的主要标本特征。反映出随着RA疾病不断进展,其病机变化存在着一个由实至虚实夹杂,再到虚的动态演变规律。关节疼痛数等主要临床症状、体征及ESR等实验室指标并不能反映活动期RA中医证候分布上的变化。
Objective：To explore the characteristics of TCM syndrome distribution on rheumatoid arthritis （RA）. Methods： 126 patients were selected for correlation research of TCM syndrome with course of the disease, rheumatoid factor （RF）, c-reactive protein （CRP）, blood sedimentation （ESR） and morning stiffness, the main clinical symptoms, physical signs, X-ray signs. Results：The difference is obviously different in the course of the disease, hands on X-ray stage. Conclusion：The trend of Xiecong Rehua is a main feature of RA pathogenesis during active stage. As long as a continuous development of RA, pathogenesis change always come with a dynamic evolution from Shi to Xu/Shi finally to Xu. Clinical symptoms of joint pain, body signs and laboratory parameters such as ESR does not reflect the TCM syndromes distribution of active RA.
