摘要
目的 探讨浙江省金华市细菌性痢疾发病规律、流行特征,为制定细菌性痢疾防控策略和措施提供科学依据.方法 采用描述性流行病学方法对金华市2007-2012年细菌性痢疾疫情资料进行分析.结果 金华市2007-2012年细菌性痢疾发病率在30.14/10万~7.89/10万之间,年均发病率为18.96/10万.发病高峰为每年6-10月份,占6年报告发病总数的61.03%.男女年均发病率居前两位的均是0岁和1~4岁年龄组;职业分布以散居儿童发病最多,占比为37.03%,男女性别比为1.33:1.分离的志贺菌对环丙沙星、诺氟沙星和头孢曲松比较敏感,对氨苄西林、头孢唑啉、复方新诺明、庆大霉素和强力霉素的耐药率都在89%以上.结论 金华市2007-2012年菌痢发病率呈逐年下降趋势,儿童和学生是防治重点人群.志贺菌属耐药与多重耐药现象比较严重,应加强志贺菌耐药性监测.
Objective To explore the incidence regularity and epidemiological features of bacillary dysentery in Jinhua City, Zhejiang Province from 2007 to 2012 so as to provide a scientific basis for formulating its prevention and control strategies and measures. Methods The method of descriptive epidemiology was used to analyze the epidemic data of bacillary dysentery in Jinhua City during 2007 - 2012. Results The incidence rates of bacillary dysentery in Jinhua City during 2007 - 2012 floated between 30.14/100,000 and 7.89/100,000, with an average incidence rate of 18.96/100,000. The peak of the yearly epidemic occurred mainly between June and October, accounting for 61.03% of the total cases reported during the six years. The top two average annual incidence rates in both males and females occurred in the groups aged below 12 months or 1 - 4 years. Scattered children accounted for the largest proportion, with a ratio of 37.03 %. And the sex ratio was 1.33 : 1. The isolated Skigella was sensitive to ciprofloxacin, norfloxacin and ceftriaxone, but resistant to ampicillin, cefazolin, compound SMZ, gentamicin and doxycycline, and the resistant rates were all over 89 %. Conclusions The incidence rates of bacillary dysentery in Jinhua City during 2007 - 2012 show the trend of gradual decrease. Children and students should be the key groups for bacillary dysentery prevention and control. The problems of antimicrobial resistance and multi- drug resistance in Shigellas species are severe; thus it is important to strengthen the surveillance of drug resistance of Shigella.
